创伤性颅内出血早期血浆D-二聚体水平对评估伤情的意义

摘　　要：目的研究早期测定创伤性颅内出血患者血浆D-二聚体水平对评估伤情的意义。方法用酶联荧光分析法(ELFA)测定病例组105例及对照组30例血浆D-二聚体水平。结果病例组血浆D-二聚体水平明显高于对照组(P<0.01)。病例组中,GCS评分越低组,血浆D-二聚体水平越高(P<0.01)。病例组血浆D-二聚体水平与GCS评分呈负相关(r=-0.874,P<0.01)。创伤性颅内出血早期血浆D-二聚体水平随头颅CT中线移位程度的加重而升高(P<0.01)。有迟发性颅内出血的患者血浆D-二聚体水平明显高于无迟发性颅内出血的患者(P<0.01)。结论创伤性颅内出血患者早期血浆D-二聚体水平是一项能反映其伤情严重程度的可靠指标。Objective To investigate the significance of level of plasma D-dimer on judging the condition in early traumatic intracranial hemorrhage.Methods The level of plasma D-dimer was detected by enzyme-linked fluorescent assay（ELFA）in 105 cases of case group and 30 cases of control group.Results The level of plasma D-dimer of case group was remarkably higher than that of control group（P0.01）.The lower the Glasgow coma scale,is the higher the level of plasma D-dimer（P0.01gets）.The level of plasma D-dimer correlated negatively with GCS（Correlation coefficients was r=-0.874,P0.01）.The greater the midline-shift distance on brain computed tomography shows,the higher the level of plasma D-dimer is（P0.01）.The level of plasma D-dimer of 21 cases with delayed traumatic intracranial hemorrhage was remarkably higher than that of non delayed traumatic intracranial hemorrhage（P0.01）.Conclusion The level of plasma D-dimer could be as a reliable indicator on judging the condition in early traumatic intracranial hemorrhage.
