不稳定型心绞痛患者血清MMP-2的含量及其与抗脂质过氧化治疗的关系

摘　　要：目的:探讨测定不稳定型心绞痛(UAP)患者血清基质金属蛋白酶-2(MMP-2)含量的意义及其与抗脂质过氧化的关系。方法:连续选择我院住院UAP患者26例(UAP组),稳定型心绞痛(SAP)21例(SAP组),另选健康体检者20例(健康对照组);以双抗体夹心酶联免疫(ELISA)法测定血清MMP-2,黄嘌呤氧化酶法测定血清超氧化物歧化酶(SOD),硫代巴比妥酸(TBA)法测定血清丙二醛(MDA)的含量。结果:(1)UAP组血清MMP-2水平明显高于SAP组和健康对照组[(54.00±7.07)ng/ml∶(39.50±4.18)ng/ml∶(35.26±1.94)ng/ml,P<0.01],SAP组血清MMP-2水平亦高于对照组(P<0.01);(2)UAP组血清MDA水平显著高于SAP组和健康对照组[(5.29±1.48)nmol/L∶(3.62±0.87)nmol/L∶(3.29±0.41)nmol/L,P<0.01],SAP组与健康对照组差别无显著性;(3)UAP组血清SOD水平明显低于SAP组和健康对照组[(86.39±4.01)nmol/L∶(93.20±4.28)nmol/L∶(100.24±3.98)nmol/L,P<0.01],SAP组血清SOD水平亦显著低于健康对照组(P<0.01);(4)相关分析显示:UAP组血清MMP-2水平与MDA水平呈正相关(r=0.481,P<0.05),血清MMP-2水平与SOD水平呈负相关(r=-0.504,P<0.05)。结论:测定血清基质金属蛋白酶-2的含量对预测不稳定型心绞痛有一定价值;抗脂质过氧化治疗可能通过抑制基质金属蛋白酶-2的高表达而有稳定粥样斑块的作用。Objective：To investigate the value of measure matrix metalloproteinase-2（MMP-2） in prediction of unstable angina pectoris（UAP） and correlation between MMP-2 and superoxide dismutase（SOD）level.Methods：A total of 26 UAP patients（UAP group） and 21 stable angina pectoris（SAP） patients（SAP group） were chosen continuously from our hospital.Another 20 healthy subjects were enrolled as healthy control group.Enzyme linked immunosorbent assay（ELISA） was used to detect MMP-2 level,xanthine oxidase method was used to measure level of SOD and thiobarbituric acid（TBA） method was applied to detect malondialdehyde（MDA） level.Results：1.The serum MMP-2 level in UAP group was significantly higher than those in SAP group and healthy control group [（54.00±7.07） ng/ml vs.（39.50±4.18） ng/ml vs.（35.26±1.94） ng/ml,P0.01],and the serum MMP-2 level in SAP group was also higher than that of control group（P0.01）;2.The serum MDA level in UAP group was significantly higher than those in SAP group and healthy control group [（5.29±1.48） nmol/L vs.（3.62±0.87） nmol/ L vs.（3.29±0.41） nmol/ L,P0.01].There was no significant difference in serum MDA level between SAP group and healthy control group.3.The serum SOD level in UAP group was significantly lower than those in SAP group and healthy control group [（86.39±4.01） nmol/ L vs.（93.20±4.28） nmol/ L vs.（100.24±3.98） nmol/ L,P0.01],and the serum SOD level in SAP group was also lower than that in healthy control group（P0.01）;4.Correlation analysis indicated that serum MMP-2 level was positively correlated with MDA level（r=0.481,P0.05） and serum MMP-2 level was negatively correlated with SOD level（r=-0.504,P0.05） in UAP group.Conclusion：Level of matrix metalloproteinase-2 may have value in predict unstable angina pectoris.Treatment for anti-peroxidation of lipid may stabilize plague by inhibiting the high expression of matrix metalloproteinase-2.
