乳头溢液306例诊断与治疗分析

摘　　要：目的探讨单纯性乳头溢液的诊断及治疗措施,明确乳腺病变与乳头溢液性质的关系。方法对我院确诊的乳头溢液306例的临床资料进行分析。结果本组术后病理检查示乳腺导管炎占32.68%,乳腺导管扩张症占28.10%,乳管内乳头状瘤或乳头状瘤病占16.67%,小叶囊性增生占15.03%,乳腺癌占3.27%,导管发育不良占2.94%,乳腺良性肿瘤占1.31%。乳腺导管炎、乳腺导管扩张症及导管发育不良中最常见的为黄色溢液分别占35.00%、29.07%和44.44%,乳管内乳头状瘤或乳头状瘤病及乳腺癌中最常见的为血性溢液,分别占43.14%和90.00%,小叶囊性增生和乳腺良性肿瘤中最常见的为浆液性溢液,分别占45.65%和75.00%。本组10例乳腺癌行乳腺癌改良根治术,2例重度乳头状瘤病行全乳切除。306例均临床治愈,随访1～3年无复发。结论乳头溢液可因多种病引起,尤其可能与乳腺癌相关,对此类患者临床医师应详细询问溢液性状,采取恰当检查方法,制定相应治疗措施,以最大限度改善预后。Objective To investigate the diagnosis,differential diagnosis and treatment of simple nipple discharge,and to identify the relationship between breast duct disease and nipple discharge.Methods The clinical data of 306 cases of nipple discharge was respectively analyzed.Results According to the pathological results,breast inflammation accounted for 32.68%,mammary duct ectasia accounted for 28.10%,intraductal papilloma or papillomatosis accounted for 16.67%,lobular cystic hyperplasia accounted for 15.03%,breast cancer accounted for 3.27%,duct dysplasia accounted for 2.94%,benign breast tumor accounted for 1.31%.Fluor flavus was the most manifested in breast inflammation（35.00%）,mammary duct ectasia（29.07%） and duct dysplasia（44.44%）.Hemal liquorrhoea was the most manifested in intraductal papilloma / papillomatosis（43.14%） and breast cancer（90.00%）.Serous discharge was the most manifested in lobular cystic hyperplasia（45.65%） and benign breast tumor（75.00%）.10 cases of breast cancer received modified radical operation,and 2 cases of severe papillomatosis received total resection of breast.All the 306 cases were cured with no recurrence during the follow up of 1 to 3 years.Conclusion Nipple discharge may be caused by many diseases and related to breast cancer.Detailed information of characteristics of liquorrhoea and proper detecting method are important in diagnosis,treatment and prognosis of the disease.
