双吻合技术联合全直肠系膜切除在低位直肠癌保肛术中的应用

摘　　要：为探讨双吻合技术（DST）联合全直肠系膜切除（TME）在低位直肠癌保肛术中的应用价值，回顾分析采用DST联合TME行低位保肛术的39例低位直肠癌资料。结果显示，39例患者均顺利完成手术，无手术死亡病例。术后发生吻合口漏3例（7．7％），吻合口狭窄2例（5．1％），吻合口出血1例（2．6％）。术后随访3个月至2年，肝转移1例（2．6％），局部复发3例（7．7％）。结果表明，DST联合TME能提高低位直肠癌保肛术的成功率，改善患者生存质量，是治疗低位直肠癌安全、有效的方法。The objective of the study was to assess the clinical value of the combined procedure of double stapling technique（DST） and total mesorectal excision（TME） in the ano-saving operation for low rectal carcinoma. The clinical data of 39 patients with low rectal carcinoma who had undergone DST and TME were analyzed retrospectively. Results showed that there was no operative death, anastomotic leakage occurred in 3 cases （7. 7%）, anastomotic stricture in 2 cases （5. 1%） and anastomotic bleeding in 1 case （2.6%） ,hepatic metastasis was found in 1 case （2.6%） and local recurrence in 3 cases（7.7%） during follow up of 3-24 months. It is concluded that the combined procedure of DST and TME can enhance the success rate of the ano-saving operation for low rectal carcinoma,improve the patients＇ life quality,thus is safe effective.
