PPH结合电刀治疗重度混合痔的临床疗效

摘　　要：为探讨吻合器痔上黏膜环切术（PPH）结合电刀治疗重度混合痔的临床疗效，回顾分析采用PPH结合电刀治疗（PPH组）或外剥内扎治疗（外剥内扎组）的重度混合痔患者的资料，对手术治疗结果进行对比分析。结果显示，两组患者手术均获成功，均无复发病例，临床疗效无明显差异，但PPH组在手术时间、住院时间、创口愈合时间及术后疼痛、出血、水肿、尿潴留、肛门控便能力、皮赘及痔核残留等方面明显优于外剥内扎术组。结果表明，PPH结合电刀治疗重度混合痔是一种安全有效的手术方式，具有手术时间短、术后恢复快、并发症少等优点，较传统的外剥内扎术具有明显的优势，值得临床推广和应用。The objective of the study was to investigate the clinical therapeutic effect of the combination therapy（PPH plus electrotome） for severe mixed hemorrhoids. Authors analysed the data from patients received the combined therapy （as treatment group,i, e PPH group） or external dissection ＆ internal ligation （as controlled group） for their severe mixed hemorrhoids,and compared their operative outcomes. As results ,the operation of both groups was performed successfully,no recurrence was found, and there was no significant difference in therapeutic effect between both groups;but in operation time,hospitalization duration, healing time of wound, as well as postoperative pain, bleeding, edema, urine retention, anal control ability for defecation, skin tag and hemorrhoids residue, and so on,PPH group was all superior to controlled group. Results show that PPH combined with electrotome for severe mixed hemorrhoids is safe and effective, and has such advantages as shorter operation time, faster recovery at postoperation,and less complication,etc,all superior to routine procedure,is worthy to be popularized and used clinically.
