摘  要:目的 通过MRI测量髌骨和髌韧带的参数,为临床诊断和治疗提供数据.方法 无髌骨损伤,伴随有或无前交叉韧带损伤患者共141例,行膝关节MRI检查.在MRI矢状面图像上测量髌骨长度和厚度以及髌韧带的长度.结果 总样本髌骨的平均长度、厚度及髌韧带长度分别是(41.1±3.6)mm、(21.4±1.7)mm、(41.5±5.2)mm,无韧带损伤组(A组)髌骨平均长度和厚度及髌韧带长度分别是(43.2±3.2)mm、(20.4±1.5)mm、(42.2±5.2)mm;前交叉韧带重建术后功能良好组(B组)患者髌骨平均长度和厚度及髌韧带长度分别是(40.3±2.8)mm、(22.6±1.3)mm、(40.4±4.3)mm;前交叉韧带重建术后恢复不良组(C组)患者髌骨平均长度和厚度及髌韧带长度分别是(42.4±2.7)mm、(20.7±1.4)mm、(45.4±4.1)mm.B、C组患者髌韧带长度差异有统计学意义,重建术后不良患者髌韧带松弛.结论 通过MRI测量髌骨和髌韧带的相关数据,为膝关节疾患的诊断及手术提供参考数据.Objective To provide the data for clinical diagnosis and treatment of knee disorders by measuring geometrical parameters of patella and patellar ligament by MRI. Methods A total of 141 patients who suffered anterior cruciate ligament(ACL) injury( or not)but no patellar injury were studied by MRI. The length and thickness of patella and the length of patellar tendon were determined on the sagittal plane films. Results The average length and thickness of patella, and the length of patellar tendon were ( 41.1± 3.6) mm, ( 21.4 ± 1.7 )mm and ( 41.5 ± 5.2 ) mm respectively. These parameters in no ACL injury group(A) were (43.2 ± 3.2 ) mm, ( 20.4 ± 1.5 )mm and (42.2 ± 5.2 )mm respectively ; those in the ACL reconstruction with good outcome group (B) were(40.3 ±2.8 ) mm, (22.6 ±1. 3 ) mm and (40.4 ± 4.3 )mm ; those in the ACL reconstruction with poor outcome group ( C ) were ( 42.4 ± 2.7 ) mm, ( 20.7± 1.4 )mm and (45.4± 4.1 ) mm. There was significant difference in patellar tendon length between group B and C. Patellar tendons were elongated in poor clinical outcome patients after ACL reconstruction. Conclusion It is a valuable method to determine the patellar parameters by MRI,which will be of great help for the diagnosis and treatment of knee disorders.
