急性心肌梗死兔原发性心室颤动早期行心前区捶击复律效果探讨

摘　　要：目的:观察急性心肌梗死兔原发性心室颤动早期行心前区捶击复律的效果。方法:180只日本长耳兔,开胸结扎冠状动脉左旋支的左室支,制备急性心肌梗死模型。术中肢导联心电监护,将急性心肌梗死并发原发性心室颤动兔随机分为2组,实验组于2min内给心前区捶击,对照组给心脏按压。观察心室颤动转归。结果:心肌梗死造模过程中并发原发性心室颤动兔共15只,实验组8只行心前区捶击,5只成功转复为窦性心律,心前区捶击复律成功率62.5%。对照组7只采取心脏按压者均死亡,复律成功率为0%。2组差异显著(P<0.05)。结论:急性心肌梗死兔原发性心室颤动早期行心前区捶击复律有效。Objective：To observe the effect of precordial thumping on the early primary ventricular fibrillation in rabbit model with acute myocardial infarction.Methods：One hundred and eighty Japanese rabbits underwent ligation of the major branch of left circumflex artery to prepare acute myocardial infarction models,while monitored by electrocardiograph.Then the models with primarily ventricular fibrillation were divided into two groups.One group was given a sharp precordial thump immediately within two minutes after the onset of ventricular fibrillation,the other group was given heart massage or precordial massage.The outcome of ventricular fibrillation was recorded.Results：Fifteen rabbits developed ventricular fibrillation on the bench after ligation of the left circumflex artery,5 rabbits were successfully defibrillated by a sharp precordial thump in 8 rabbits（62.5%）.The other 7 rabbits died after heart massage or precordial massage.The difference was statistically significant between 2 groups（P0.05）.Conclusions：Thumping of the precordium is effective on the early primary ventricular fibrillation in rabbit model with acute myocardial infarction.
