摘 要:目的应用国际参考方法对MICROTEST1自动血沉仪进行校准,建立其量值溯源性,同时调查不同性别和年龄生物参考区间。方法选取50例抗凝血标本,分别用MICROTEST1自动血沉仪和国际血液学标准化委员会(ICSH)推荐的参考方法魏氏法进行检测,校准仪器,建立MICROTEST1自动血沉仪量值溯源性。663例健康体检人员按性别、年龄分组,对各组数据进行统计学分析,确定ESR参考区间调查的分组原则及计算生物参考区间的方式。结果MICROTEST1自动血沉仪校准后与魏氏法有较好的相关性(r=0.982),配对t检验表明两种方法检测结果差异无统计学显著性意义(P〉0.05)。按性别、年龄分组,将男性分为20~50岁,〉50岁两个年龄段;女性分为20~40岁,41~50岁,〉50岁三个年龄段,采用百分位数法分别计算不同性别和年龄段血沉的生物参考区间。男性50岁及以下年龄段血沉参考区间≤23.1mm/h,50岁以上≤38.8mm/h;女性40岁及以下血沉参考区间≤25.4mm/h,41~50岁≤37.5mm/h,50岁以上≤45.3mm/h。结论应用ICSH参考方法校准MICROTEST1自动血沉仪,可实现其测量结果的可溯源性,同时可促进不同临床实验室ESR检测结果互认。ESR的生物参考区间受性别、年龄等影响较大,通过建立可靠的生物参考区间,为临床提供了参考依据,更好地发挥血沉在临床诊断中的作用。Objective To establish measurement traceability and investigate biological reference interval for MICRO TEST1 ESR analyzer calibrated by reference method of ICSH (International Committee for standardization in hematology). Methods 50 EDTA-K2 anticoagulated blood samples were deteced by MICRO TEST1 ESR analyzer and westergren method. The reference method for measuring the ESR proposed by ICSH. Data obtained by this two methods were used to calibrate the instrument and to establish measurement traceability. 663 healthy cases were divided into differ- ent groups according to sex and age. Statistical analysis were used to determined how to group and establish biological ref- erence interval. Results Linear regression analysis comparing MICRO TEST1 and ICSH reference method yielded satis- factory correlations (r= 0. 982). Paired student t test showed that there wasnot significant different between this two meth- ods. The cases were divided intodifferent groups according to sex and age and biological reference interval were established using percentiles. In male,there were two groups which were 18~50 years(≤23.1 mm/h) and 〉50 years (438.8 ram/ h). In female,there were three groups which were 18~50 years(≤25.4 mm/h) ,41~50 years(≤37.5 mm/h) and 〉50 years(≤45.3 ram/h). Conclusion Calibrated by ICSH reference method,MICRO TEST1 ESR analyzer established mea- surement traceability and promoted clinical laboratory results mutual accreditation. Biological reference interval of ESR were affected by sex and age. The establishment of reliable reference interval could be consulted by clinic and play a better role in clinical diagnosis.
