DHS在老年股骨转子间骨折治疗应用中的体会

摘　　要：目的探讨DHS在老年股骨转子间骨折治疗中的治疗效果以及注意事项。方法收集2005年以来骨科收治的老年股骨转子间骨折患者41例,术前调整患者内科疾病直至能耐受手术,均在全身麻醉下完成手术,术中平卧位,患侧垫高约15度,行大腿上段外侧纵行切口,整复骨折,在导向器下钻入定位导针,C形臂确定满意后扩孔,置入空心加压螺钉,套上钢板,分别钻孔和螺钉固定,创面充分止血,放置引流,缝合伤口,术毕常规抗感染以及内科调理等,统计所有41例患者手术前后VAS评分变化以及患者出院时髋关节活动度以评价手术效果。结果 DHS治疗后髋关节功能活动达到基本正常或以上者为34例,占82.9%,患者VAS评分明显较治疗前降低,手术效果明显。结论 DHS是一种治疗股骨转子间骨折的好方法,对高龄患者充分的术前准备加上良好的业务技能,术中细致认真,术后密切观察十分重要。Objective To investigate the effects, and precautions of DHS for the treatment of intertrochanteric fractures. Methods Since 2010, our hospital orthopedic intertrochanteric fractures treated in 41 patients ,adjustment until able to tolerate surgery, all 41 cases were done under general anesthesia, intraoperative contralateral lateral position, lateral femoral longitudinal incision, fracture alignment, drill into positioning guide pin, C-arm confirmed, reaming, into coarse grain hollow screw and compression screw, adjust the position, fixed, bleeding, suturing, bandaging, of surgery conventional anti-infective and medical conditioning and so on, statisticaled all 41 patients VAS score changes before and after surgery and at discharge in patients with hip range of motion to evaluate the effect of surgery. Results DHS activities after treatment of hip joint function to normal or above is 34 cases, accounting for 82.9%, VAS score were significantly lower than that before. Conclusion DHS is a good way for intertrochanteric fractures, with adequate preoperative preparation good business skills, careful and meticulous surgery, close observation after surgery is important, in particular with elderly patients.
