立芷雪加缩宫素预防剖宫产出血的疗效观察

摘　　要：目的:观察立芷雪加缩宫素预防剖宫产出血的疗效。方法:选择剖宫产病人300例,随机分为缩宫素组和立芷雪+缩宫素组,每组各150例。缩宫素组:胎儿娩出后,立即予肌内注射缩宫素10U,同时20U缩宫素加入液体静脉滴注。立芷雪+缩宫素:术前30min肌内注射1 KU立芷雪,胎儿娩出后同缩宫素组。结果:用立芷雪+缩宫素组出现产后出血的例数及出血量均明显少于缩宫素组(P<0.01);术前缩宫素组和立芷雪+缩宫素组的出凝血时间(BT、CT)、凝血酶原时间(PT)及部分凝血酶原时间(APTT)分别是(0.64±0.12)s、(4.13±0.74)s、(12.6±1.2)s、(26.9±2.2)s;(0.69±0.17)s、(4.20±0.75)s、(12.2±1.4)s、(27.3±3.2)s;术毕两组BT、CT、PT、APTT分别是(0.68±0.21)s、(3.89±0.65)s、(11.6±1.4)s、(27.8±1.6)s;(0.62±0.27)s、(4.24±0.51)s、(12.4±1.3)s、(27.7±2.4)s。术前、术毕两组的凝血功能比较差异均无显著性(P>0.05)。结论:立芷雪加缩宫素对预防剖宫产出血具有一定的价值,并且对机体系统凝血功能无明显影响。Objective：To observe the effect of reptilase and oxytocin for preventing hemorrhage of caesarean birth.Methods：300 cases of caesarean birth were randomly divided into oxytocin group and the reptilase plus oxytocin group.The oxytocin group was immediately given oxytocin 10U by intramuscular injection after fetus delivery and simultaneously 20U oxytocin by intravenous drip.The reptilase plus oxytocin group received intramuscular injection of reptilase lkU at 30 min before caesarean birth and after fetus delivery the treatment was same to that in the oxytocin group.Results：Compared with the oxytocin group,the number and quantity of caesarean birth hemorrhage in reptilase plus oxytocin group were obviously less.But the hemoglutination function in the two groups had no significant difference before and after operation.BT,CT,PT and APTT before operation were（0.64±0.12）s,（4.13±0.74）s,（12.6±1.2）s and（26.9±2.2）s in the oxytocin group and（0.69±0.17）s,（4.20±0.75）s,（12.2±1.4）s and（27.3±3.2）s in the reptilase plus oxytocin group,while which after operation were（0.68±0.21）s,（3.89±0.65）s,（11.6±1.4）s and（27.8±1.6）s in the oxytocin group and（0.62±0.27）s,（4.24±0.51）s,（12.4±1.3）s and（27.7±2.4）s in reptilase plus oxytocin group respectively.Conclusion：Reptilase plus oxytocin have certain value in preventing hemorrhage of caesarean birth,which do not affect the hemoglutination function.
