早产儿脑室出血临床与细胞外基质观察

摘  要:目的探讨早产儿脑室出血(Intraventricularhemorrhage,IVH)与侧脑室壁和脉络丛细胞外基质成份的关系。方法10例早产儿IVH和10例早产儿非IVH死亡病例,每例均取侧脑室壁和脉络丛两份标本,分别用抗IV型胶原抗体和抗LN抗体标记,观察计算阳性显色单位面积。结果IVH组细胞外基质LN和IV型胶原阳性产物平均单位面积小于非IVH组,也即两组间LN和IV型胶原含量差异具有统计学意义。结论侧脑室室管膜下和脉络丛微血管细胞外基质发育不成熟是早产儿易患IVH的形态学基础。To investigate the relationship between extracelluar matrix, both inchoroid plexus and the wall of lateral ventricles, and intraventricular hemorrhage in pretermneonates. Methods Speciments of total 20 cases which were taken from lateral ventricular walland charoid plexus in 10 preterm neonates who died of intraventricular hemorrhage and the other 10preterm neonates who died of non-intraventricular hemorrhage were marked with antibodies of typeIV collagen and laminin. The positive staining area arround microvessels was studied. ResultsThe average area of positive products of these two extracellular matrix in IVH group was less thanthat in non-IVH group (P<0. 05). Conclusion The choroid plexus and subependymal microvessels of preterm neonates are short of type IV collagen and laminin. This may account for immature ofbasement membrane of microvessels and occurrence of IVH in preterm neonates.
