曲美他嗪对非体外循环冠脉搭桥术患者CK、CK-MB及cTnI的影响

摘　　要：目的:观察曲美他嗪对非体外循环冠状动脉(冠脉)旁路移植术(off-pump coronary artery bypass grafting,OP-CAB)患者肌酸激酶(creation kinase,CK)和肌酸激酶同工酶(creation kinase-MB,CK-MB)以及心肌肌钙蛋白I(car-diac troponin I,cTnI)的影响,探讨其心肌保护作用机制。方法:将103例于我院择期行OPCAB的冠心病患者随机分为曲美他嗪组(52例)和对照组(51例)。分别于术前、术后6 h、术后12 h和术后24 h抽取静脉血,测定血清CK、CK-MB及cTnI。结果:两组患者临床特征及桥血管情况差异无统计学意义。曲美他嗪组术前CK比较,差异无统计学意义,在术后6 h、12 h、24 h明显低于对照组(P<0.01)。曲美他嗪组术前CK-MB比较,差异无统计学意义,在术后6 h、12 h和24 h明显低于对照组(P<0.01)。曲美他嗪组术前cTnI比较,差异无统计学意义,在术后6 h、12 h和24 h明显低于对照组(P<0.01)。结论:曲美他嗪可明显降低OPCAB患者CK、CK-MB和cTnI的释放。Objective： To observe the effects of Trimetazidine on serum level, CK, CK-MB and cTnI of the patients under- went off-pump coronary artery bypass grafting （OPCAB） and discuses the mechanism of the protection on myocardium. Methods： One hundred and three patients who underwent off-pump coronary artery bypass grafting were randomly divided into Trimetazidine group （52 cases） and control group （51 cases）. CK, CK-MB and cTnI of preoperative, postoperative six hours, postoperative twelve hours and postoperative twenty-four hours by intravenous exsanguinate were analyzed. Results： There were no significant difference between clinical characterizes and grafts of two groups. There was no significant differ- ence between serum CK level before surgery of two groups. The serum CK level of Trimetazidine group was lower than that of control group after surgery six hours, twelve hours and twenty-four hours, respectively （P〈0.01）. No significant difference between serum CK-MB level of two groups before surgery, the serum CK-MB level of Trimetazidine group was lower than that of control group after surgery six hours, twelve hours and twenty-four hours,respectively （P〈0.01）. No difference between serum cTnI level of two groups before surgery, the serum cTnI level of Trimetazidine group was lower than that of control group after surgery six hours, twelve hours and twenty-four hours, respectively （P〈0.01）. Conclusion： Trimetazidine may re- duce the release of the serum level of CK,CK-MB and cTnI of the patients underwent OPCAB.
