连续性血液净化治疗重症胰腺炎的临床体会

摘　　要：目的探讨连续性血液净化(continuousbloodpurification,CBP)治疗重症胰腺炎(severeacutepancreatitis,SAP)的临床疗效。方法 2006年1月至2009年12月间收治的38例SAP患者在常规治疗的基础上加CBP,观察患者治疗前后生命体征、血液生化指标、炎症因子、氧合指数。结果治疗后患者的心率,呼吸,氧合,血肌酐,血淀粉酶、脂肪酶,炎症因子均较治疗前有了显著好转,且48h效果要好于24h。治疗对血压的影响不大。38例中34例治愈,3例死亡,1例因脑死亡放弃治疗。结论 CBP能改善和维持SAP患者重要脏器功能,显著降低了SAP死亡率,改善了SAP预后,是治疗SAP新途径。Objective To investigate clinical therapeutic effect severe acute pancreatitis therapied with continuous blood purification.Methods In 2006.1 ～2009.12,38 case of SAP patient in our hospital were therapied with CBP on the basis of conventional therapy,their vital signs, blood biochemical parameters,inflammatory factors,and oxygenation index were observed before and after treatment.Results TO compare with before therapy,After therapy,heart rate,breathing,oxygenation,serum creatinine,serum amylase,lipase,inflammatory cytokines of SAP patient have been significantly ameliorate,and therapeutic efficacy of 48h is better than 24h.Treatment had little effect on blood pressure.34 cases were cured,3case died and 1case of brain death gave up treatment in 38 cases.Conclusion CBP can improve and maintain the function of important organs in patients with SAP,significantly cut down mortality,ameliorate prognosis of SAP.CBP is a new approach to therapy SAP.
