摘  要:目的:比较下鼻甲部分切除术与微波热凝术治疗慢性肥厚性鼻炎的疗效及术后反应情况。方法:对1996年8月至2003年12月对慢性肥厚性鼻炎患者行下鼻甲部分切除术143例及下鼻甲微波热凝术236例进行总结,比较手术操作、术后反应及疗效。结果:两种治疗方法总有效率差异有统计学意义(P<0.05),微波热凝术组下鼻甲肿胀、鼻塞持续时间及伪膜消失时间均较下鼻甲部分切除手术组长(P<0.05),术后疼痛及血涕持续时间较下鼻甲部分切除手术组短(P<0.05)。结论:下鼻甲部分切除术治疗慢性肥厚性鼻炎疗效肯定,对下鼻甲骨肥厚以及合并的鼻中隔偏曲可同时处理,下鼻甲微波热凝术治疗慢性肥厚性鼻炎操作简单、疼痛轻、出血少,但鼻甲肿胀及愈合时间较长,不能同时处理骨性病变。Objective: To compare the effects and postoperative reaction between partial inferior turbinectomy and microwave thermocoagulation on patients with chronic hypertrophic rhinitis.Methods: From Aug of 1996 to Dec of 2003,143 and 236 cases with chronic hypertrophic rhinitis were performed partial inferior turbinectomy and microwave thermocoagulation,respectively.Then the data about the operating,postoperative reaction and curative effects between the two groups were compared.Results:There was a significant difference in the effective rates between two treatment methods.The time of turbinate turgidity,nasal obstruction,and the loss of pseudomembranous in microwave thermocoagulation group was significantly longer than that in partial inferior turbinectomy group,but the degree of postoperative ache and the time of nasal hemorrhage in microwave thermocoagulation group were significantly shorter than those in partial inferior turbinectomy group.Conclusion: The partial inferior turbinectomy may have some advantages in treatment effects,and it can also treat some complications which lead to nasal obstruction at the same time.The method of microwave thermocagulation is simple,less painful and bleeding,but the time of turbinate turgidity and cure is longer.
