经前路颈椎手术后单次注射帕瑞昔布钠镇痛效果观察

摘　　要：目的观察经前路颈椎手术后单次静注帕瑞昔布钠和盐酸曲马多的镇痛效果。方法选择60例择期经前路颈椎手术患者,采用随机数字法将患者随机分成帕瑞昔布钠(P组)和盐酸曲马多两组(T组),手术结束前30分钟,P组(n=30)静脉注射帕瑞昔布钠40 mg,T组(n=30)静脉注射盐酸曲马多2 mg/kg。所有患者在手术结束后1、4、8、12及24小时进行疼痛视觉模拟评分(VAS),记录呼吸抑制、恶心、呕吐、嗜睡、皮肤瘙痒等及术后24小时内使用镇痛药情况。结果两组患者术后VAS均小于3分,两组比较差异无统计学意义(P>0.05);P组嗜睡、头晕及恶心呕吐分别有0、1、2例,T组则分别为3、4、6例,两组比较差异有统计学意义(P<0.05),T组恶心、呕吐、头晕及嗜睡等不良反应较显著。结论经前路颈椎手术后短期应用帕瑞昔布钠具有良好的镇痛效果,不良反应较少。曲马多镇痛效果良好,但患者恶心、呕吐及嗜睡发生率较高。Objective To assess the analgesic effect of Parecoxib Sodium and Tramadol Hydrochloride in patients undergoing anterior cervical surgery.Methods Sixty patients scheduled for anterior cervical surgery were randomly allocated to either group P（Parecoxib Sodium）or group T（Tramadol Hydrochloride）.Parecoxib Sodium 40 mg or Tramadol Hydrochloride 2 mg/kg was injected 30 minutes before the end of surgery.Postoperative pain was assessed by Visual Analogue Scale（VAS）in l,4,8,12,and 24 hours after surgery.Side effect such as respiratory depression,nausea,emesia,drowsiness,itch of skin and the analgesic use within 24 hours were also assessed.Results Both two groups got low VAS scores（less than 3）.There was no significant difference between the two groups（P0.05）.In group P,the incidence of drowsiness,dizziness,nausea and vomiting was 0,1/30 and 2/30 respectively,while in group T,it was 3/30,4/30,and 6/30 respectively.Difference in side effect between the two groups was significant（P0.05）.Conclusion The study suggests that Parecoxib Sodium injected before the end of anterior cervical surgery provides satisfactory analgesic effect with lower side effect.Compared with Parecoxib Sodium,Tramadol Hydrochloride can provide satisfactory analgesic effect with higher incidence of nausea,vomiting and drowsiness.
