老年腰椎间盘突出并椎管狭窄症的手术治疗

摘　　要：目的探讨全椎板切除术式治疗腰椎间盘突出并椎管狭窄症的中长期疗效。方法对40例老年腰椎间盘突出并椎管狭窄症患者行后路全椎板切除治疗,年龄65～75岁,平均(69.5±3.5)岁,病程3d～5年,术后随访24～60个月,平均45个月,回顾性分析临床资料,按照Oswestry功能障碍指数(Oswestrydisabilityindex,ODI)设计问卷随访,对临床效果进行评分并行统计学分析。结果该术式总体优良率92.5%。结论全椎板切除术为腰椎间盘突出并椎管狭窄症彻底减压创造良好条件,无论是单节段或双节段病变,均能提高临床疗效。全椎板切除术是治疗单节段或双节段老年腰椎间盘突出并椎管狭窄症的安全、有效方法。Objective To evaluate the middle-term and long-tern therapeutic effect of holovertebral plate discectomy to treat lumbar disc herniation and stenosis of aged.Methods A total of 40 patients with lumbar disc herniation and stenosis were involved in this study.All patients were treated with holovertebral plate discectomy,with the mean age of（69.5±3.5） years,ranged from 65 to 75 years.All patients were followed up 24-60 months,with average 45 months.The clinical results were evaluated according to Oswestry disability index（ODI） method.Results The excellent and good rates of this group were 92.5%.Conclusion The holovertebral plate discectomy place a good ground for posterior decompression of lumbar disc herniation and stenosis of aged,either single segment or double segments,the effect can be improved.The holovertebral plate discectomy is safe and effective method to treat lumbar disc herniation and stenosis of aged,either single segment or double segments.
