摘 要:目的研究4种氟喹诺酮类药物对鲍氏不动杆菌耐药突变株的选择能力,比较其防耐药变异能力。方法肉汤法富集1010CFU/ml鲍氏不动杆菌和34株临床分离菌,采用平板稀释法对加替沙星、左氧氟沙星、帕珠沙星和环丙沙星对鲍氏不动杆菌的MIC99、暂定防耐药变异浓度及防耐药变异浓度(MPC)进行测定。结果加替沙星、左氧氟沙星、帕珠沙星、环丙沙星对鲍氏不动杆菌的MPC分别为1.2、2.8、5.6和12.8μg/ml;细菌耐药选择指数分别为3.4、4、8和16μg/ml;加替沙星、左氧氟沙星、帕珠沙星和环丙沙星对34株鲍氏不动杆菌临床分离菌MPC值分别为0.5、1、8和4μg/ml,选择指数分别为4、4、8和8μg/ml。结论加替沙星和左氧氟沙星限制鲍氏不动杆菌耐药突变菌株的选择能力强于环丙沙星和帕珠沙星。OBJECTIVE To measure the mutant prevention concentrations (MPC) in vitro of fluoroquinolones for acinetobacter baurnannii and compare the capacity of fluoroquinolones to prevent the resistant mutant strain. METHODS The A. baurnannii strain ATCC19606 and 34 clinical isolates were enriched in broth,and the bacterial concentrations were adjusted to 101% colony forming units per milliliter. The minimal inhibitory concentration (MIC), MIC for 99 of input cells (MIC99), provisional MPC (MPCpr) and MPC of gatifloxacin, levofloxacin, pazufloxacin,and ciprofloxacin for A. baurnannii were determined by agar plates dilution method. RESULTS The MPC of gatifloxacin,levofloxacin,pazufloxacin and ciprofloxacin for A. baumannii strain ATCC19606 were 1.2, 2.8,5.6 and 12.8 μg/ml,and the MPC/MIC99 were 3.4, 4,8, 16 respectively. The MPC for 90 of the isolates (MPCpr90)of gatifloxacin, levofloxacin, pazufloxacin and ciprofloxacin, for 34 A. baurnannii clinical isolates were 0.5,1,8 and 4μg/ml, and the MPCpr90/MIC90 were 4,4,8 and 8, respectively. CONCLUSION The capacity of gatifloxacin and levofloxaein for restricting the selection of A. baumannii resistant mutants is stronger than that of pazufloxacin and ciprofloxacin.
