摘　　要：目的了解重庆市男男性行为（MSM）人群同性无保护肛交情况及其影响因素。方法 2006年和2007年7-9月分别进行了2次横断面调查,通过面对面访谈方式进行问卷调查。收集人口学特征、艾滋病知识、高危行为及既往性传播感染信息,并采血进行梅毒和人类免疫缺陷病毒（HIV）检测。结果 2次招募合格调查对象1 434人,平均年龄为27.0岁,未婚占78.8%,高中及以上文化程度者占86.1%;65.7%MSM自我认同为同性恋,近6个月同性无保护肛交发生率为63.6%;19.9%有异性性行为。同性无保护肛交的独立相关变量包括大龄（OR=1.02,95%CI=1.0～1.03）、未接受性病相关服务（OR=1.4,95%CI=1.1～1.7）、曾有性传播疾病相关症状（OR=1.7,95%CI=1.2～2.3）及未参与卖同性性行为（OR=3.0,95%CI=1.9～4.7）;艾滋病知识得分越高者发生同性无保护肛交越少（OR=0.9,95%CI=0.8～1.0）。结论重庆市MSM人群同性无保护肛交发生率高,多性伴及异性性行为多见。HIV和梅毒感染率较高。Objective To examine the prevalence and predictors of unprotected anal sex among men who have sex with men（MSM） in Chongqing municipality,China.Methods Two consecutive cross-sectional surveys were conducted among MSM in three districts in June-July,2006 and 2007.Information on demographic characteristics,HIV knowledge,sexual and drug use behavior,HIV/STD related services and self-reported symptoms of sexually transmitted diseases（STDs）were collected.Results A total of 1434 MSM were recruited in the two surveys.The average age of the participants was 27.0 year.Of all participants,22.2% were married,86.1% received high school or higher level education,65.7% were self-identified as hmosexual and 31.3% as bisexual.The rate of unprotected anal sex in the previous 6 months was 63.6%.Factors independently associated with unprotected sex included old age（odds ratio=1.02,95% confidence interval：1.0-1.03）,having no commercial sex work with another male partner（OR=3.0,95%CI：1.9-4.7）,receiving no STDs service（OR=1.4,95%CI：1.1-1.7）,self-reported STDs（OR=1.7,95%CI：1.2-2.3）,and high level of HIV/AIDS knowledge（OR=0.9,95%CI：0.8-1.0）.Conclusion The prevalence of unprotected anal sex among MSM in Chongqing was very high.Multiple sex partners and heterosexual behaviors were common.HIV and syphilis prevalence had reached a high level.
