肺表面活性物质治疗新生儿呼吸窘迫综合征的临床观察

摘　　要：目的观察肺表面活性制剂(PS)在治疗新生儿呼吸窘迫综合征(NRDS)的疗效和安全性。方法对我院48例NRDS患儿在给予常规综合治疗的基础上分成观察组(加固尔苏、猪肺磷脂,PS的一种)、对照组(未加PS),观察治疗前后两组患儿肺功能和肺泡充气程度的变化。结果观察组临床有效率96.4%,死亡率、机械通气时间和胸部X线严重性明显低于对照组(P<0.05),观察组用药后30min患儿血PaO2明显升高,血PaCO2明显降低,血pH由偏低变为正常,用药后48h,胸部X线片显示患儿肺野透亮度明显改善。结论固尔苏对NRDS有显著疗效,能安全、有效、迅速改善患儿的氧气交换和肺部病变。Objective To observate the efficacy and safety of pulmonary surfactant（PS） in the replacement therapy for neonatal respiratory distress syndrome（NRDS）.Methods 48 NRDS infants were divided into two groups.28 cases received curosurf which is one of the PS and 20 cases didnt received curosurf.All of the other supportive treatments were same in the both groups The changes of the lung function and lung aeration after the therapy in both groups were kept under observation.Results In the therapeutic group,the clinical effective rate is 96.4%.The death rate,ventilative supporting time were lower significantly than those in the control group.After 30 minutes of the administration in the treatment group,PaO2 significantly increased,PaCO2 significantly decreased（P0.05）.After 48 hours of the administration,chest X-ray showed a significant improvement in lung aeration.Conclusion Curosurf has definite effect on neonatal respiratory distress syndrome,it could rapidly and effectively improve the lung function and lung aeration.
