糖化血红蛋白检测对原发性高血压合并冠心病住院患者的临床价值

摘　　要：目的探讨糖化血红蛋白检测对原发性高血压合并冠心病但未诊断糖尿病住院患者的临床价值。方法对165例原发性高血压合并冠心病但未诊断糖尿病住院患者进行空腹血糖(fasting plasma glucose,FPG)、糖化血清蛋白(glycosylated serum protein,SFA)、糖化血红蛋白(hemoglobin A1c,HbA1c)检测,并分析患者平均血糖水平和血糖升高者比例。结果 165例原发性高血压合并冠心病住院患者FPG、SFA和HbA1c的平均水平分别为5.87±1.59mmol/L、2.66±0.57mmol/L、6.30±1.36(%)。达到糖尿病空腹血糖诊断标准(≥7.0mmol/L)和糖化血红蛋白诊断标准(≥6.5%)者比例分别为15.2%(25/165)和25.5%(42/165),两者差异有显著性差异(χ2=5.412,P=0.020)。达到糖尿病前期空腹血糖水平(≥5.7,<7.0mmol/L)和糖化血红蛋白水平(≥5.7%,<6.5%)者比例分别为28.5%(47/165)和40.0%(66/165),两者差异显著(χ2=4.858,P=0.028)。结论对于原发性高血压合并冠心病但未诊断糖尿病住院患者,用HbA1c水平评估患者糖代谢异常较FPG更敏感。Objective The objective is to evaluate the value of hemoglobin A1c test for hospitalized non-diabetic patients with essential hypertension and coronary heart disease.Methods The fasting plasma glucose(FPG),glycosylated serum protein(SFA),hemoglobin A1c(HbA1c) were detected in 165 cases of essential hypertension with coronary heart disease but without other diseases.The average blood glucose levels and the ratios of patients with elevated blood glucose were analyzed.Results The average levels of FPG,SFA and HbA1c were 5.87±1.59mmol/L,2.66±0.57mmol/L and 6.30±1.36(%),respectively,in the population investigated.The ratio of the patients meeting diagnostic criteria for diabetes in term of HbA1c(≥6.5%) was significantly higher than that of the FPG(≥7.0mmol/L).(25.5% Vs 15.2%,χ2= 5.412,P=0.020).The ratios of the patients with FPG≥5.7 but<7.0mmol/L and with HbA1c≥5.7%,but<6.5% were significantly different(40.0% Vs 28.5%,χ2=4.858,P=0.028).Conclusion It is more sensitive to assess abnormal glucose metabolism in hospitalized non-diabetic patients with essential hypertension and coronary heart disease with HbA1c test than that of FPG.
