摘 要:目的探讨氢质子磁共振波谱(proton magnetic resonance spectroscopy,1H-MRS)联合术中磁共振(in-traoperative magnetic resonance imaging,iMRI)导航在颅内病变穿刺活检的应用。方法使用兼具有1H-MRS功能的1.5 T术中磁共振系统,并配有导航计算机成像系统。26例颅内病变患者,先使用1H-MRS分析病灶组织生化及代谢情况,然后在iMRI导航引导下根据病变不同部位代谢情况对病变相应区域进行穿刺活检。结果 26例患者穿刺成功,25例病变组织获得组织病理学诊断。所有病例术中及术后均未出现明显并发症如严重出血、新发神经功能障碍等。结论 1H-MRS分析能提供病变的代谢信息,并帮助确定最佳活检取材点,iMRI导航能精确定位,并在术中明确取材位置和排除出血等并发症,1H-MRS联合iMRI导航在颅内病变活检是一种精准、有效、安全的技术。Objective To assess the clinical effectiveness of the combined application of ^1H-MRS and iMRI navigation in needle biopsy of intracranial lesions. Methods ^1H-MRS was used to analyze the metabolism of the lesions and select the best biopsy point. A 1.5T iMRI and a navigation imaging system were used to identify the pathological lesion in 26 brain biopsy procedures. Results Needle Biopsies were successfully performed in all 26 cases and pathological diagnosis were made in 25 patients. None of the patients suffered from significant complications such as severe bleeding or neurologieal deficit. Conclusions ^1 H-MRS is vel7 helpful in selecting the best biopsy lesion point and iMRI can guide biopsy accurately. The combined application of ^1 H-MRS and iMRI navigation is an accurate, effective and safe technique for needle biopsy of intracranial lesions.
