超声引导经皮穿刺中心静脉导管置入的临床价值

摘　　要：目的探讨超声引导在经皮穿刺中心静脉导管置入术中的临床价值。方法 316例慢性肾病血液透析患者随机分为两组:超声引导经皮穿刺导管置入组(U组)169例,传统盲法经皮穿刺导管置入组(T组)147例,比较两组一次穿刺成功率、穿刺时间和并发症。结果 U组一次置管成功率为97.6%,高于T组的83.0%(P<0.05);U组穿刺时间(9.8±2.2)min,低于T组的(15.2±3.8)min(P<0.05);T组穿刺并发症8.8%,高于U组的1.2%(P<0.01)。结论超声引导经皮穿刺中心静脉导管置入术能显著提高置管成功率,缩短置管操作时间,减少并发症的发生。Objective To evaluate the clinical value of ultrasound （US） - guided percutaneous puncture in central venous catheterization. Methods Three hundred and sixteen patients with chronic nephrosis and underwent hemodialysis were divided into US -guided pereutaneous catheterization group （US group, 169 cases） and traditional pereutaneous catheteration group （T group, 147 cases）. The one time successful rate, duration and complication of puncture were compared between groups. Results The catheterization successful rate of US group and T group was 97.6% and 83.0%, respectively （ P 〈 0. 05 ）, the puncture duration of US group and T group was （9.8± 2.2 ） min and （ 15.2 ±3.8 ） min, respectively （ P 〈 0.05 ）, the ineidenee rate of complications in US group and T group was 8.8% and 1.2% , respectively （ P 〈 0.01 ）. Conclusion Ultrasound - guided percutaneous puncture can significantly improve the successful rate of central venous catheteration, reduce the opration time of catheter placement and decrease the oecurrence of complications.
