摘  要:目的:探讨CT血管成像(CTA)在判断急慢性期下肢深静脉血栓形成(DVT)中的临床价值。方法:回顾性分析37例经DSA证实为DVT的病例,分析其在CT增强图像上形态学表现,测量平扫时轴位图像上血栓CT值及标准差,分别做两样本均数t检验。结果:急性期DVT18例,增强图像上多呈管腔中央类圆形低密度充盈缺损,范围广,常累及全下肢,血栓处管腔明显扩张,患肢软组织肿胀明显。慢性期DVT19例,增强图像上多呈管腔内不规则形状的低密度充盈缺损,节段性分布,血栓处管腔无明显扩大,可有局部管腔狭窄闭塞,管壁不规则增厚,侧枝循环增多,浅静脉增粗。平扫时急慢性期血栓CT值平均数分别为59.26HU和37.42HU,两者差异有统计学意义(t=18.29,P<0.05),平扫时急慢性期血栓标准差平均数分别为12.46和8.34,两者差异有统计学意义(t=4.27,P<0.05)。结论:CTA对显示急慢性期血栓有一定价值,可为临床治疗方案的制定提供依据。Objective:To assess the value of CT angiography(CTA) in differentiating acute or chronic deep venous thrombosis(DVT).Methods:The CTA data of 37 patients with DVT confirmed by DSA were retrospectively studied.The appearance of thrombus,vessels,soft tissue and superior veins were reviewed in enhanced CT.The CT value and standard deviation of thrombus were measured in non-enhanced CT.The data were compared using independent samples t-test.Results:18 patients with acute DVT showed round-like low density filling defect in dilated veins,usually long extent.The soft tissue was swelling significantly.19 patients with chronic DVT showed irregular low density filling defect in veins.The lumens were not dilated but may stenosis even obstructed.Lateral branches and superior veins increased.The average CT value of acute and chronic DVT were 59.26HU and 37.42HU(t=18.29,P0.05),and the average standard deviation were 12.46 and 8.34 respectively(t=4.27,P0.05).Conclusion:To some extent,CTA can differentiate acute or chronic DVT,and then guide the treatment.
