摘 要:目的联合应用磁共振弥散张量成像(DTI)、感兴趣区(ROI)、基于体素的图像分析技术(VBA)研究高血压患者的脑白质损伤。方法对受试者进行临床和神经心理评估、头颅MRI平扫。13例受试者(高血压组7例、对照组6例)接受DTI扫描,所得DTI影像数据经与标准模板配准后,行全脑全体素分析及ROI,从而确定两组各向异性分数(FA)有差异的脑区。结果高血压组右侧顶叶白质、左侧枕叶白质、右侧枕叶白质FA值显著低于对照组,差异有统计学意义(P<0.001)。结论与MRI平扫T2加权成像及ROI比较,DTI联合VBA技术可以更敏感、更准确的发现高血压相关的脑白质损伤。Objective To measure microstructural differences in the brains of participants with hypertension compared with an age-matched control group using magnetic resonance diffusion tensor imaging(DTI)technique with ROI and fully automated image analysis tools.Methods A standardized clinical and neuropsychological evaluation was conducted on each subject.Thirteen participants(7 participants with hypertension,6 with normal blood pressure)underwent magnetic resonance imaging(MRI)-based DTI.To control for the effects of anatomical variation,diffusion images of all participants were registered to standard anatomical space.Voxel-by-voxel comparisons showed significant regional reductions of fractional anisotropy(FA)in participants with hypertension compared to controls in white matter regions.Results Significantly decreased FA values measurements(P0.001)were observed in the right parietal,left temporal,right occipital white matter in participants with hypertension.Conclusions Compared with T2-weighted MRI and ROI,diffusion tensor imaging and voxel-based analysis could more sensitively and precisely find Leukoaraiosis of hypertension.
