老年住院患者深静脉血栓形成的回顾性分析

摘　　要：目的探讨老年住院患者深静脉血栓形成(DVT)的特点及预后。方法回顾性分析2004年～2010年住院治疗,临床及影像学诊断为DVT的133例老年患者的临床资料。结果 133例老年DVT患者的年龄为65～96(75.8±7.6)岁,其中男83例(62.41%),女50例(37.59%)。老年患者深静脉血栓主要累及下肢(84.96%),其中以左下肢最为常见占48.12%,上肢及颈内静脉血栓占15.04%。高血压、动脉硬化及恶性肿瘤是最常见的前3位并发疾病。112例患者(84.21%)应用抗凝治疗,19例(14.29%)接受了溶栓治疗,2例(1.50%)实施了手术取栓术,15例(11.28%)植入了下腔静脉滤器。133例患者共发生肺栓塞16例(12.03%),死亡19例(14.29%)。多元Logistic回归分析显示增龄及感染是住院老年患者DVT死亡的独立相关因素。结论老年住院患者DVT主要发生在下肢,但上肢静脉血栓形成发病有增多趋势,增龄及感染是老年患者DVT死亡的高危因素。Objective To analyze the clinical features and outcome in hospitalized elderly patients with deep vein thrombosis（DVT）.Methods One hundrend and thirty-three patients aged 65 years and older with DVT,admitted to PLA General Hospital from January 2004 to January 2010,were analyzed retrospectively.Results Totally 133 patients were enrolled into the study.The mean age was 75.8±7.6y.Eighty three（62.41%）were male.Most（84.96%）cases of DVT occur in the lower extremities,among which left lower extemities accouted for 48.12%.Upper extremity DVT was found in 20 patients（15.04% of DVT）.The most common risk factors and concurrent disease was hypertension,followed by atherosclerosis and cancer.112 out of 133（84.21%）patients were treated by anticoagulation,19 cases（14.29%）received thrombolytic therapy,2 cases underwent embolectomy,and 15 patients（11.28%）received an inferior vena cava filter.Of the 133 patients with DVT,16（12.03%）developed pulmonary embolism（PE）,and 19（14.29%）died during hospitalization.Multivariate logistic regression analysis showed advanced age and infection were significantly associated with increased mortality risk in patients with DVT.Conclusions DVT mostly involves the lower extremities in hospitalized patients,whereas there has been an increased incidence of upper extremity deep vein thrombosis.Advanced age and infection are independently risk factors for death among aged patients with DVT.
