摘  要:目的分析呼吸机相关性肺炎(VAP)病原菌鲍曼不动杆菌的分布特点及耐药性,以指导临床合理用药。方法对明确行气管插管或气管切开机械通气>48h的66例患者的气管内抽吸物(ETA)进行细菌定量培养及抗菌药物敏感性测定。结果回顾性分析66例VAP患者ETA培养出病原菌152株,鲍曼不动杆菌55株(36.2%);鲍曼不动杆菌首选抗菌药物是头孢哌酮舒巴坦和亚胺培南;鲍曼不动杆菌对常见抗菌药物的耐药率呈上升趋势,并呈多药耐药;头孢哌酮舒巴坦耐药率最低为54.5%,对亚胺培南耐药率达74.5%,且呈上升趋势。结论动态监测鲍曼不动杆菌的耐药性变化,有助于临床合理应用抗菌药物。Objective To investigate the distribution and drug resistance characteristic of acinetobacter baumannii in patients with ventilator-associated pneumonia(VAP) and direct clinical doctors to select antibiotics correctly.Methods The endotracheal aspirates of 66 patients received tracheostomy and mechanical ventilation for more than 48 hours was cultured with a quantitative method and their antibiotic susceptibility was detected.Results Among 152 strains of isolated pathogens in 66 patients,acinetobacter baumannii occupied 36.2%.Acinetobacter baumannii were susceptible to cefoperazone sulbactam and imipenem.Acinetobacter baumannii showed high rate of resistance to routine antimicrobial agents and even multipled drug resisttance.The rate of antimicrobial resistance of acinetobacter baumannii to cefoperazone sulbactam was 54.5%,the resistant rates to imipenam was 74.5% and showed a remarkable uptrend.Conclusion To monitor changs of antimicrobial resistance of acinetobacter baumannii,it is useful to select antibiotics correctly for treatment of VAP.
