鲍氏不动杆菌耐药率分析

摘　　要：目的探讨不同病房鲍氏不动杆菌耐药率。方法对2009年重症监护病房(ICU)、呼吸内科和烧伤整形科送检标本参照《全国临床检验操作规程》(第3版)的方法培养,细菌鉴定采用MicroScan-Walk Away-40细菌鉴定/药敏分析系统,按照美国2009年CLSI判断标准进行,并对结果进行χ2检验。结果 2009年1月1日-12月31日3个病房共分离出鲍氏不动杆菌66株,其中ICU 34株,占51.5%;呼吸内科24株,占36.4%;烧伤整形科8株,占12.1%;主要来源于痰、创面分泌物、静脉血和腹水,鲍氏不动杆菌在ICU和烧伤整形病房表现为对多种常用抗菌药物耐药。结论鲍氏不动杆菌在不同病房的耐药率不同,临床医师应该根据各科室耐药特点合理使用抗菌药物。OBJECTIVE To investigate the drug susceptibility pattern of Acinetobacter baumannii.METHODS The specimens derived from intensive care unit,internal medicine of respiration and department of burn and plastic surgery during of hospital stay in 2009 were included in the study.The culture of bacteria were carried out by National Clinical Laboratory Operating Instruction（the third edition） and the identification as well as susceptibility test of bacteria were detected by microscan-walkaway-40 system and judged by criteria of Clinical and Laboratory Standard Institute of America（CLSI）2009.Datas are analyzed by chi-square test.RESULTS Among total 66 strains of A.baumannii separated from Jan 1,2009 to Dec 31,2009 in three hospital rooms.34 strains（51.5%） were isolated from intensive care unit,24 strains（36.4%） isolated from respiration department,8 strains（12.1%） isolated from department of burn and plastic surgery.The samples came mainly from sputum,wound secretion,venous blood and hydroperitoneum.Susceptibility test demonstrated that A.baumannii isolated from ICU as well as department of burn and plastic surgery were multi-drug resistant to commonly used antibiotics.CONCLUSION The drug resistance rate of A.baumannii is different in various hospital wards and these different antimicrobial susceptibility patterns are necessary for the clinician to formulate a rational antibiotic policy.
