老年高血压肾气亏虚证证候量化诊断标准指标赋权方法探讨

摘　　要：目的通过老年高血压病肾气亏虚证诊断标准中指标权重系数的建立,探讨中医证候量化诊断标准指标赋权的合理方法。方法对《老年高血压病肾气亏虚证宏观诊断量表》采用主客观联合赋权法,先利用德尔菲(Delphi)法确定量表各条目主观权重系数,再对老年高血压患者300例样本进行探索性因子分析确定客观权重系数,然后将两种赋权方法得出的某一指标的权数相乘,最后进行归一化处理。结果量表各条目整数化权值分别为头晕7、腰酸8、膝软8、健忘4、听力减退4、发脱3、齿摇4、夜尿频多7、尿有余沥5、神疲8、乏力9、气短7、嗜卧2、诸症遇劳加重7、舌质淡白3、脉沉5、脉细5、脉弱5。结论联合赋权法对中医证候量化具有可行性和科学性,更能反映临床实际。Objective To find out a reasonable method to weight the quantification indices in TCM syndrome diagnosis through establishing the weight coefficients in diagnosing the kidney deficiency syndrome of senile hypertension patients.Methods The subjective and objective weighting method was adopted to determine the subjective and objective weight coefficients for the Macroscopic Diagnosis Scale of the Kidney Deficiency Syndrome of Senile Hypertension Patients.First,the Delphi method was used to determine the subjective weight coefficient of various items of the scale,then an exploring factorial analysis was done for 300 sample patients to determine the objective weight coefficient.The weight of a certain index obtained from the two were multiplied,and finally,they were processed for normalization.Results The weight values of the items of the scale were dizziness 7,lumbar soreness 8,knee weakness 8,forgetfulness 4,hearing decline 4,hair loss 3,teeth loosening 4,micturition frequency 7,urine dribbling 5,listlessness 8,tiredness 9,shortness of breath 7,somnolence 2,symptoms getting worse when tired 7,pale tongue 3,deep pulse 5,thready pulse 5,and weak pulse 5 respectively.Conclusion The weight values of the target combination of each index in the diagnostic standard of the kidney deficiency syndrome of senile hypertension patients were established,indicating that the combined weighting method in the quantification diagnosis of TCM syndromes is feasible and scientific.
