视黄醇结合蛋白4超敏C反应蛋白与代谢综合征痰瘀辨证关系研究

摘　　要：目的:探讨视黄醇结合蛋白4(RBP4)、超敏C反应蛋白(hs-CRP)与代谢综合征(MS)痰瘀辨证之间的关系。方法:将60例MS患者中医辨证分为痰浊瘀阻型与非痰浊瘀阻型两组,另设正常对照组30例。测定3组患者的血清RBP4、血清hs-CRP、空腹血糖(FPG)、空腹胰岛素(FINS),胰岛素抵抗指数(HOMA-IR),并测定身高、体重、腰围(WC)、血压,计算体重指数(BMI)。结果:痰浊瘀阻组BMI、WC、RBP4、hs-CRP、HOMA-IR各值均高于非痰浊瘀阻组(P<0.05),显著高于正常组(P<0.01)。非痰浊瘀阻组hs-CRP高于正常组(P<0.05),BMI、WC、RBP4、HOMA-IR显著高于正常组(P<0.01)。RBP4与WC呈正相关(r=0.014),与BMI、hs-CRP、HOMA-IR密切关系(r值分别为0.001、0.006、0.001)。hs-CRP与BMI、HOMA-IR呈正相关(r值分别为0.023、0.038)。结论:RBP4、hs-CRP与痰瘀关系密切。明确了痰瘀在MS中重要性。RBP4、hs-CRP可作为中医辨证诊治代谢综合征的客观指标。Objective：To investigate the relationship between retinol binding protein 4（RBP4）,high-sensitive c-reactive protein（hs-CRP） and symptoms and signs of phlegm-blood stasis of metabolic syndrome（MS）.Methods：sixty cases of MS were divided into groups of coagulation of phlegm-blood stasis type and non-coagulation of phlegm-blood stasis type.Thirty healthy people were taken as normal control group.RBP4,hs-CRP,fasting plasma glucose（FPG）,fasting insulin（FINS） were detected and Homeostasismodel assessment of insulin resistance（HOMA-IR）was calculated.Height,weight,waist circumference（WC）,blood pressure and BMI were measured.Result：BMI,WC,RBP4,hs-CRP,HOMA-IR in coagulation of phlegm-blood stasis group higher than those of the non-coagulation of phlegm-blood stasis group（P0.05）and were significantly higher than those of the healthy persons（P0.01）.Hs-CRP in non-coagulation of phlegm-blood stasis group higher than those of the healthy persons（P0.05）.BMI、WC、RBP4、HOMA-IR in non-coagulation of phlegm-blood stasis group are significantly higher than those of the healthy persons（P0.01）.RBP4 was positively correlated with WC（r=0.014）.There was a markedly positive correlation between RBP4 and BMI,hs-CRP,HOMA-IR（r=0.001,0.006,0.001）.Hs-CRP was positively correlated with BMI,HOMA-IR（r=0.023,0.038）.Conclusions：There was a markedly correlation between RBP4,hs-CRP and phlegm-blood stasis.It is definite that phlegm-blood stasis paly an important role in MS.RBP4、hs-CRP can be used as an objective indicator for TCM syndrome differentiation in diagnosis and treatment of MS.
