足月新生儿动脉导管自然闭合情况的研究

摘　　要：目的探讨足月新生儿动脉导管自然关闭的情况,以及是否进行早期人工干预的必要性。方法将2007年3月～2007年12月间正常产检,孕期超声筛查胎儿心脏正常并在该院分娩的1 437名足月新生儿纳入研究,在新生儿出生后24～96 h内进行超声心动检查,对其中发现的单纯动脉导管开放的新生儿,在生后3月复查超声心动了解其闭合情况并电话随访其发育情况。结果 1437例足月新生儿中发现单纯动脉导管开放的318例,生后24 h动脉导管59.3%处于开放状态,48 h、72 h、96 h的比例分别为27.1%、15.2%、9.0%。11例失访,304例在生后3月内自然闭合,诊断PDA 3例,其发生率为1∶475。男∶女为1∶2。结论足月新生儿动脉导管在生后96h后仍有自然闭合的可能,如无临床症状,无须过度干预,但应注意随访,确定其闭合情况。【Objective】 To learn the status of the patent ductus arteriosus in term births and to investigate the necessity of early artificial intervention.【Methods】 During March 2007 to December 2007,1 437 term newborn infants were examined by Doppler echocardiography after 24 hours to 96 hours of the delivery,and the infants with isolated persistence of the arterial duct were followed in the next 3 months to ensure if the ductus arteriosus had closed.【Results】 Persistence of the arterial duct was detected in 318 term newborn infants.In the first 24 hours 59.3% ductus was still open,in 48 hours,72 hours,96 hours the scale was 27.1%,15.2% and 9%.During the follow up,304 cases spontaneously closed without any treatment,only 3 cases were diagnosed patent ductus arteriosus PDA.The incidence of PDA is 1 ∶ 475,with a male to female ratio of 1 ∶ 2.【Conclusions】 In term newborn infants ductal patency may be detected after 96 hours of age,having no significant symptoms.If asymptomatic,it shouldn＇t be think abnormal and needn＇t any treatment but follow up.
