摘  要:目的评估超脉冲等离子体联合钬激光技术直视下尿道内切开术治疗尿道狭窄和闭锁的有效性和安全性。方法应用超脉冲等离子体联合钬激光技术结合输尿管镜及电切镜行直视下尿道内切开术治疗26例男性尿道狭窄和尿道闭锁,其中前尿道狭窄14例,闭锁3例;后尿道狭窄6例,闭锁3例。对临床资料进行回顾分析,分析手术要点。结果 25例手术1次成功,1例术后3个月出现再狭窄行再次手术后有效,临床治愈率为80.8%,有效率为19.2%。随访26~36个月,26例患者排尿均通畅,无性功能障碍和尿失禁发生。结论超脉冲等离子体联合钬激光技术直视下尿道内切开术治疗尿道狭窄和尿道闭锁安全有效,该方法对适合行尿道内切开术的病例临床疗效满意。【Objective】 To evaluate the efficacy and safety of optical internal urethrotomy with super-impulse plasmakinetic body combined with Holmium laser for clinical management of urethral stricture and obliteration.【Methods】 A retrospective analysis was done on clinical data of 26 cases clinically diagnosed as having urethral stricture and obliteration hospitalized and treated by super-impulse plasmakinetic body combinated with Holmium laser under endoscope from June 2007 to April 2008.There were 13 patients with anterior urethral stricture and 6 with posterior urethral strictures,3 patients with anterior urethral obliteration and 3 with posterior urethral obliteration,respectively.In addition,techniques in operation were discussed.【Results】 All the patients were treated successfully,25 cases were cured after operation,1 case experienced second operation was cured due to recurrence after 3 months.The clinical curative rate in this series was 80.8% and the efficacious rate 19.2%.All the patients had normal urination with 26~36 months follow-up,and revealed no postoperative sexual dysfunction and incontinence.【Conclusions】 Optical internal urethrotomy with super-impulse plasmakinetic body combined with holmium laser for clinical management of urethral stricture and obliteration is the safe and effective for those patients suitable to optical internal urethrotomy.
