腹部小切口辅助阴式子宫切除的术式探讨

摘　　要：目的探讨腹部小切口辅助的阴式子宫切除术的临床应用价值。方法 2007年2月—2010年2月在枣庄市立医院行腹部小切口辅助阴式子宫切除术68例为研究组,与同期阴式子宫切除术56例,开腹子宫切除术33例进行对照,对多项临床指标进行分析。结果研究组手术时间较腹式组和阴式组短(P<0.05);术中出血少于腹式组和阴式组(P<0.05,P<0.01);术后排气时间较腹式组早(P<0.01),与阴式组无差异;术后下床活动时间腹式组早(P<0.01),与阴式组无差异;术后病率低于腹式组(P<0.01),与阴式组比较无差异;术后住院天数较腹式组少(P<0.01),与阴式组无差异。结论腹部小切口辅助阴式子宫切除术具有手术时间短、术中出血少、住院时间短、创伤小的优点,具有临床应用价值。Objactive To study the clinical efficacy of small abdominal incision assisted vaginal hysterectomy.Methods A total of 157 patients with benign uterine and ovarian disease,who underwent TAH,TVH or small abdominal incision assisted vaginal hysterectomy in our hospital from February 2007 to March 2010,were retrospectively analyzed（68 paitents with small abdominal incision assisted vaginal hysterectomy as study group,56 cases with TVH and 33 cases with TAH）.Results The study group had significantly short operative time than TVH and TAH（P〈0.05）.Blood loss in study group was significantly less than that in TAH（P〈0.05）.The pyrexua rate was significantly lower in study group than that in TVH and TAH（P〈0.05,P〈0.01）.Hospital stay was shorter in study group than that in TAH（P〈0.05）.Conclusions Small abdominal incision assisted vaginal hysterectomy had shorter operative time,less blood loss,lower pyrexua rate,and shorter hospital time.It can reduce the patient’s economic burden,There is more value for clinical application in this operative way.
