摘  要:目的了解临床分离病原菌的分布及对抗菌药物的耐药状况。方法收集2005年1月—2009年12月临床各科室送检的标本,按"全国临床检验操作规程"要求进行操作,按美国临床实验室标准化研究所(CLSI)标准判读结果。结果共检出4571株病原菌,其中2355株革兰阴性杆菌(51.5%);1182株革兰阳性球菌(25.9%);前5位依次是大肠埃希菌、克雷伯氏菌属、凝固酶阴性葡萄球菌、金黄色葡萄球菌、铜绿假单胞菌。检出肠杆菌科细菌1636株,产超广谱β-内酰胺酶(ESBLs)370株(22.6%,370/1636)。MRSA检出率40.9%(122/298),MRSE检出率48.4%(212/438),VRE 4.2%(10/236)。亚胺培南和哌拉西林/他唑巴坦对革兰阴性杆菌敏感率最高,氨苄西林、哌拉西林、头孢唑啉、复方新诺明耐药率最高。结论抗菌药物临床合理应用工作不能懈怠,合理应用抗菌药物是减少耐药菌株发生的重要措施。Objective To understand the distribution of clinical isolates of pathogens and the antimicrobial drug resistance.Methods Collected from January 2005 to December 2009 submission of clinical specimens of various departments,by The National Clinical Test Regulation of Operation,according to Clinical Laboratory Standards Institute(CLSI) standard to interpret the results.Results Totally 4571 pathogens were detected,of which 2355 gram negative bacilli(51.5%);1182 Gram-positive cocci(25.9%);First 5 followed by Escherichia coli、Klebsiella、coagulase-negative Staphylococcus、Staphylococcus aureus、Pseudomonas aeruginosa.Detected in Enterobacteriaceae 1636,producing extended spectrum β-lactamases(ESBLs) 370 strains(22.6%,370/1636).MRSA detection rate of 40.9%(122/298),MRSE detection rate of 48.4%(212/438),VRE 4.2%(10/236).IMP and TZP against gram-negative bacilli were most sensitive,AMP,PIP,FZN,SXT resistance rate was the highest resistance rate.Conclusions Clinical use of antibiotics can not slack off work,reasonable application of antibiotics is to reduce the drug-resistant strains is an important measure.
