摘 要:目的探讨单纯性肥胖儿童体块指数(BMI)与胰岛素敏感性及脂肪肝的关系。方法 126例2~14岁的单纯性肥胖儿童及30例年龄、性别相匹配的健康正常体重儿童分成5组,分别检测体块指数、空腹血糖(FBG)、空腹血胰岛素(FINS)及肝脏B超,并计算血糖/胰岛素比值(G/I比值)、胰岛素抵抗指数(HOMA-IR)和胰岛β细胞分泌指数(HOMA-IS)。结果肥胖各组FBG、G/I比值与对照组比较差异无统计学意义(P>0.05),而FINS、HOMA-IR、HOMA-IS差异有统计学意义(均P<0.05)。B超检查显示肥胖组儿童有16例并发脂肪肝,发病率为12.75%,正常对照组无1例脂肪肝发生(P<0.01)。结论单纯性肥胖儿童BMI与胰岛素敏感性及脂肪肝密切相关,肥胖儿童需早期干预和长期随访。Objective To explore the relationship between body mass index(BMI),insulin sensitivity and the incidence of fatty liver in simple obesity child.Methods 126 children age from 2 to 14 with simple obesity and 30 health-y controls with age and sex matched were collected and divided into five groups.Body mass index,free blood glucose(FBG),free insulin(FINS) and ultrasonography of liver were monitored.The ratio of fasting glucose/insulin(G/I ratio),homeostasis assessment model for insulin resistance(HOMA-IR) and homeostasis assessment model for insulin secretion(HOMA-IS) were calculated.Results There was no significant difference in FBG and G/I ratio between obesity and those in control(P0.05).However,FINS,HOMA-IR and HOMA-IS in obesity were significant higher than those in control(P0.05).Ultrasonography of liver showed that there 16 children with fatty liver in obsity,and there were no one with fatty liver in control,and the incidence of fatty liver in obesity was 12.75%.There was significant difference between two groups(P0.01).Conclusion BMI is closely related to insulin sensitivity and the incidence of fatty liver in obesity child.It needs of long follow-up and early intervention for obese children.
