肛痛消软膏防治痔术后肿痛的临床疗效及药效性研究

摘　　要：目的探讨肛痛消软膏防治混合痔术后肿痛及促进愈合的临床疗效及机理。方法①临床观察:将256例混合痔术后患者随机分为治疗组(128例)和对照组(128例),分别于手术结束及便后换药时予肛痛消软膏及太宁乳膏敷于创面上。②动物实验:通过热板法观察肛痛消软膏的镇痛作用;采用二甲苯引起的小鼠耳廓肿胀观察肛痛消软膏的消肿作用;通过二度烫伤家兔观察肛痛消软膏促进创面愈合作用。结果①临床观察:术后0.5小时、第10天换药后0.5小时,两组间疼痛VAS评分比较均无显著性差异(P>0.05),术后6小时～第7天换药后0.5小时两组间疼痛VAS评分及术后防治肛缘水肿、促进创面愈合比较均有显著性差异(P<0.05)。②动物实验:肛痛消软膏对热传导引起的疼痛反应有明显的拮抗作用;对小鼠耳廓肿胀有明显的抑制作用;能明显缩短家兔烫伤创面愈合时间。结论肛痛消软膏具有良好的镇痛、消肿、促进创面愈合的作用,是一种安全的中药制剂。Objective To investigate the clinical efficacy and mechanism of Gangtongxiao Ointment on Swelling and pain control and healing improvement in Post-Operational of Mixed Haemorrhoids. Method ①clinical study：Divide 256 patients into treatment group and control group randomly. Treatment group was given the Gangtongxiao Ointment after operation and dressing, and control group was given the ointment of Training.②Animal experiment： Hot plate test was used to investing the effect of analgesia; mouse auricle swelling test caused by dimethylbenzene was used to observe the effect of Gangtongxiao ointment on detumescence ; rabbits with secondary burn were used to investigate the effect of Gangtongxiao ointment on restitution of the wound, skin stimulus test, dermal sensitivity test and the acute toxicity testing were observed. Result ①clinical study in 0.5 hours after the operation , and in the first 0. 5 hours after the dressing 10 days later, VAS pain score between the two groups were not significantly different（ P 〉 0.05 ）. From 6 hours after the operation to the first 0.5 hours after the dressing 7 days later, VAS pain score, The edema control of anal verge ,and the healing improvement between the two groups were significantly different（P 〈 0.05 ）. ②Animal experiment the remission of the pain caused by the hot conduction, the inhibition of the mose auricle swelling and the decurtation of heal time were observed in the tests of Gangtongxiao ointment. Conclusion Gangtongxiao ointment has good effect on analgesia, detumescence, restitution of the wound. It is a safe Chinese drugs preparation.
