AgNOR在不同类型子宫内膜增生症中的定量研究及意义

摘　　要：目的探讨核仁组成区嗜银蛋白(AgNOR)在不同类型子宫内膜增生症中的数量差别及对预后的评价。方法采用银染法染色,检测子宫内膜增生症200例:正常增生、单纯增生、复杂增生和不典型增生各50例,分别计数组织中细胞核内AgNOR的定量值,并对其进行形态学观察。结果不典型性增生的子宫内膜细胞核内AgNOR颗粒的数量、大小、形态和在胞核内的位置与其他三组有所差别;AgNOR颗粒数比较:正常增生组间比较P>0.05;正常增生组、单纯增生组与复杂增生组比较P<0.05;正常增生组、单纯增生组与不典型性增生组比较P<0.01;复杂增生组与不典型性增生组比较P<0.05。结论AgNOR颗粒的数量可为临床治疗和随访提供量化依据,可以作为子宫内膜增生预后的评价指标。Objective To study the value of quantitative research of AgNOR in different types of endometrial hyperplasia.Methods 200 patients who were diagnosed as endometrial hyperplasia were divided into normal hyperplasia group,simple hyperplasia group,complex hyperplasia group and atypical hyperplasia group,with 50 cases in each group.Quantitative research and morphometric analysis of AgNOR were taken in different endometrial tissues with silver-staining technique.Results There were differences in dimensions,shape,localization in cell of AgNOR among atypical hyperplasia group and the other three groups.AgNOR count had no statistical differences in normal hyperplasia group（1.52±0.31,range 1.22-2.36,cutoff value2.36）,and simple hyperplasia group（1.89±0.76,range 1.31-2.89,cutoff value〈2.89）.In complex hyperplasia group,AgNOR count（3.45±0.26,range 3.15-4.82,cutoff value〉3.15） increased significantly（P〈0.05） than that in normal hyperplasia group and simple hyperplasia group;in the atypical hyperplasia group,AgNOR count（4.12±0.34,range 3.71-5.29,cutoff value 3.71）was higher than that in complex hyperplasia group（P〈0.05）;AgNOR count in the atypical hyperplasia group is significantly higher than those in normal hyperplasia group and simple hyperplasia group（P〈0.01）.Conclusion The quantitative research and morphometric analysis of AgNOR were correlated to the different types of endometrial hyperplasia,and it could provide evidence for the treatment to different types of endometrial hyperplasia and could be used as a prognostic index in different types of endometrial hyperplasia.
