下腰椎侧方入路中相关静脉的临床解剖学研究

摘　　要：目的研究下腰椎侧方入路中相关静脉的应用解剖学特点,为该手术入路提供解剖学依据。方法观测21具成人尸体标本L3、L4节段静脉、髂腰静脉(iliolumbar vein,ILV)和腰升静脉(ascending lumbar vein,ALV)的出现率、走行、分支、分布及与周围结构的毗邻关系。结果 L3、L4节段静脉、ILV和ALV出现率分别为90.48%、85.71%、69.05%和54.76%,其汇入点、外径及形态均存在较大的变异。与L3、L4静脉相比,ILV和ALV变异更为显著,其缺失率高,形态学变异较大。结论下腰椎侧方静脉的走行及解剖学形态具有一定规律性,在下腰椎侧方手术入路中,可有效避开或牵开这些结构,避免术中损伤。Objective To study anatomic characteristics of veins and provide anatomic data in the lower lumbar lateral surgical approach.Methods 21 human cadavers were investigated to record the frequency,course,branches,distribution and relationship of L3,L4 segment veins,iliolumbar veins and ascending lumbar vein.Results Frequency of L3,L4 segment veins,iliolumbar veins and ascending lumbar vein were 90.48%,85.71%,69.05% and 54.76%,their abouchement,diameter and morphology had marked variability.Compared with L3,L4 segment veins,ILV and ALV had greater varieties.They showed miss rate highly and variation greatly of morphology.Conclusions The course and anatomic morphology of lateral veins of lower lumbar had unvarying regularity,so the lateral approach of lower lumbar could efficiently get out of the way or these anatomic structures to avoid intraoperative injury.
