丙氨酰谷氨酰胺对脓毒症患者营养免疫调节和抗炎症反应的作用

摘　　要：目的研究丙氨酰谷氨酰胺对脓毒症患者营养状况、免疫功能及急性炎症反应的调理作用。方法将74例脓毒症患者随机分为治疗组38和对照组各36例。对照组仅给予常规治疗治疗组在此基础上给予丙氨酰谷氨酰胺10g加入复方氨基酸250ml中,静滴,1次/d,连续10d。分别测定两组患者治疗前后血清前白蛋白(PA)、白蛋白(ALB)、转铁蛋白(TF)、免疫球蛋白(IgG)、免疫球蛋白(IgA)、C反应蛋白(CRP)。结果 (1)治疗组血清前白蛋白、转铁蛋白等营养参数较对照组明显改善;(2)治疗组中IgG含量明显升高(P<0.05);(3)治疗组C反应蛋白及急性生理和慢性健康评分(APACHEⅡ)均较对照组下降(P<0.05)。结论脓毒症患者及时补充丙氨酰谷氨酰胺能够改善患者营养及免疫功能,减少有害或过度的炎症反应,有利于病情的控制,从而提高救治率。Objective To study the alanyl-glutamine nutritive condition,immune function and acute inflammation of toning effect on septic patients.Methods The hospital acquired 74 patients with septic,who were randomized in two groups：the treatment group in 38 cases and controlled group in 36 cases.The control group limited to conventional treatment,the treatment group in this basis,giving alanyl-glutamine 10 g join compound amino acids 250 ml static drops once daily for 10 days.Two groups of patients were measured before and after the treatment of serum albumin （PA）,former albumin （ALB）,transferrin （TF）,immunoglobulin （IgG）,immunoglobulin （IgA）,C-reactive protein （CRP）.Results （1）The treatment group serum albumin,transferrin,before the nutrition such as the control parameters is obviously improved.（2） The treatment group was obviously increase in IgG（P0.05）.（3）The treatment group C-reactive protein and APACHEⅡ were relatively controls decline.Conclution Septic patients of seasonable compensatory alanyl-glutamine can improve patients nutrition and immune function,reduce harmful or excessive inflammation,be helpful for disease control,so as to improve the cure rate.
