摘  要:目的研究生物力学在骨盆截骨术中的应用。方法选择50例先天性髋关节发育不良的患者,女性32例(64.0%),男性18例(36.0%),分析比较50例患者的生物力学髋臼负重区域角度和髋关节骨性关节炎发展的关系,分析在骨盆截骨术中如何得到最佳的治疗效果。结果髋臼负重区域角度与总髋关节关节间隙变化呈正相关(r=0.285,P=0.045),同时存在线性回归关系(t=2.058,P=0.045),Y=-1.755+0.468X。由方程得出当X=3.75时Y=0。结论在实施骨盆截骨术时,将髋臼负重区域角度控制在3.75°时术后髋关节关节间隙变窄最小、治疗效果最好。Objective To investigate the biomechanics of pelvic osteotomy.Methods Fifty patients with developmental dysplasia of the hip,32 females(64.0%)and 18 males(36.0%),were enrolled.The patients aged from 30 to 73 years,average 55.4 years.The angle of weight-bearing zone and the changes of joint space were measured.Then the follow-up results were evaluated and analyzed.Results Angle of the weight-bearing zone was related with the changes of total hip joint space(r=0.285,P=0.045).There was a linear relationship between the angle of weight-bearing zone and the changes of total joint space(t=2.058,P=0.045),and the linear equation was Y=-1.755+0.468X.If X=3.75,then Y=0.Conclusion When pelvic osteotomy is performed,if the angle of weight-bearing zone is 3.75 degrees then the narrowing of postoperative joint space is the smallest and the outcome will be better.
