摘  要:目的:探讨隐性骶椎裂对尿道外括约肌功能的影响及尿道外括约肌肌电图(EMG)检查对神经源性膀胱的临床意义。方法:30例经放射学证实隐性骶椎裂患者进行尿道外括约肌定量EMG测定。结果:尿道外括约肌均呈轻、重不同的神经源性损害EMG,表现为运动单位电位平均时限增宽,多相电位百分率增多,部份记录到自发性电位、束颤电位、巨大电位以及在尿道外括约肌记录的骶神经反射潜伏时延长。结论:马尾神经损害是致尿道外括约肌功能异常的直接原因,尿道外括约肌EMG检查能明显提高隐性骶椎裂的亚临床阳性率,有助于神经源性膀胱患者的早期诊断,EMG是评价骶神经与尿道外括约肌功能的有效方法。To explore the effects of hemirachischisis on the sphincter function and the Clinicalsignificance of external urethral sphincter EMG in neurogenic bladder. Methods: Sphincter quantitative EMG wasdetermined in 30 patients with hemirachisis confirmed by radiography. Results: EMG of external urethral sphinctershowed mild or severe neurogenic lesions. The EMG finding were as follows: The mean duration of the motor unitpotentials was prolonged and the percentage of the spontaneous potentials and polyphasic potentials increased.Fasciculation voltage and giant potentials were recorded in some patients. The latency of sacral evoked response wasprolonged as well. Conclusions: The coccygeus nerve impairment might be the direct cause of inducing the abnormalfunction of external methral sphincter.The external urethral sphincter EMG could aparently increase the subclinicalpositivity rate of hemirachischisis, and might be helpful in the early diagnosis of neurogenic bladder. EMG might be aneffective method to evaluate the funetion of sacral nerve and external urethral sphincter.
