胰岛素对继发性治疗失效NIDDM病人血压和血糖及体质量影响

摘　　要：目的观察胰岛素对口服降糖药继发性治疗失效的２型糖尿病（ＮＩＤＤＭ）病人血糖、血压、体质量的影响。②方法将３０例口服降糖药继发性失效的ＮＩＤＤＭ病人随机分为２组，对照组继续分组前的治疗，治疗组加用胰岛素，治疗０．５年，观察两组病人血压、血糖及体质量的变化。③结果治疗组治疗后血压增高，血糖降低，体质量增加，与治疗前比较差异有显著性（ｔ＝２．８１～３．９１，Ｐ＜０．０５，０．０１）；对照组相应指标无显著变化（ｔ＝１．７７～１．８７，Ｐ＞０．０５）；治疗后两组各指标变化比较，差异有极显著性（ｔ＝３．０７～５．５６，Ｐ＜０．０１）。④结论胰岛素对口服降糖药继发性治疗失效的ＮＩＤＤＭ病人有降低血糖，升高血压，增加体质量的作用。Objective\ To assess the effect of insulin on blood pressure, blood glucose and body weight in non insulin dependent diabetic(NIDDM) patients with secondary treatment faliure.\ Methods\ 30 NIDDM patients with secondary failure to oral hypoglycemic agents were randomly divided into two groups( n =15). Group A was given insulin treatment in adition to the previous treatment, Group B continued the previous treatment. Both groups were followed up for six months. Body weight,blood glucose and blood pressure were recorded.\ Results\ There was a significant increase in systolic and diastolic blood pressure and body weight and a significant decrease in blood glucose( t=2.81-3.91, P <0.05,0.01) in group A after insulin treatment. Group B demonstrated no significant change in blood pressure, blood glucose and body weight( t =1.77-1.87, P >0.05). The changes of blood pressure, blood glucose and body weight before and after treatment in the two groups were significantly different ( t=3.07-5.56,P <0.01).\ Conclusion\ This study showed that insulin treatment for patients of NIDDM with secondary faliure was associated with significant elevation of systolic and diastolic blood pressure and body weight and decrement of blood glucose.
