动脉硬化患者臂踝脉搏波传导速度、臂踝指数影响因素分析及与中医脉象的相关性

摘　　要：目的分析臂踝脉搏波传导速度(baPWV)、臂踝指数(BAI)的影响因素,探讨其与中医脉象的关系。方法收集体检者244例,排除下肢动脉有闭塞者12例,根据baPWV分为正常组(87例)、轻度硬化组(63例)和硬化组(82例),应用全自动动脉硬化测定仪测定双侧的baPWV及BAI,同时记录体检者年龄、病史、血压、血脂、体质指数和脉象,采用直线相关分析和Logistic回归分析确定影响动脉弹性的因素及与中医脉象的相关性。结果 3组体检者收缩压、舒张压、平均压、脉压差随着baPWV值的增高而呈上升趋势,各组间差异具有统计学意义(P<0.01),血脂水平及体质指数各组之间无统计学差异(P>0.05),糖尿病史、高血压史、吸烟史、饮酒史各组间差异有统计学意义(P<0.01)。Logistic回归分析显示baPWV影响因素为年龄、收缩压、糖尿病史,BAI影响因素为年龄、糖尿病史(P<0.05)。反映动脉硬度由弱到强的脉象(弱脉组、平脉组、弦脉组)双侧baPWV均逐渐增高,各组间差异均有统计学意义(P<0.01),与脉管的粗细相关的脉象(细脉组、平脉组)双侧BAI逐渐升高,各组间差异均有统计学意义(P<0.01)。结论年龄、收缩压、糖尿病史是早期动脉粥样硬化的影响因素,baPWV、BAI与脉象表现密切相关。Objective To analyze the influencing factors of brachial-and-ankle pulse wave velocity（baPWV）and brachial-ankle indices（BAI）in order to study their correlations with TCM pulse conditions.Methods Totally 244 subjects from physical examination were collected and 12 of them with arterial occlusion of lower limbs were excluded.The subjects were divided into a normal group（87 cases）,a mild sclerosis group（63 cases）,and a sclerosis group（82 cases）according to baPWV.The baPWV and BAI of lower limbs of both sides were determined with an automatic arteriosclerosis surveyor,simultaneously,the general data including age,medical history,blood pressure,blood fat,body weight index,and the pulse conditions were recorded.The linear correlation analysis and the Logistic regression analysis were carried out to determine the influencing factors on arterial elasticity and their correlation with the TCM pulse conditions.Results The systolic pressure,diastolic pressure,average pressure,and arterial differential pressure of the three groups showed a tendency to be increasing along with the increase of baPWV,and the difference among the groups was statistically significant（P0.01）.There was no difference in blood fat and constitutional index among groups（P0.05）.There was difference in diabetes history,hypertension history,smoking history,and alcohol drinking history among groups（P0.01）.Logistic regression analysis showed that the influencing factors of baPWV were age,systolic pressure,and diabetes history,and those of BAI were age and diabetes history（P0.05）.The pulse conditions that reflecting the degree of arteriosclerosis from weak to strong（weak pulse group,normal pulse group,string-taut pulse group）of baPWV of both sides were gradually advanced with a significant difference among groups（P0.01）.The pulse conditions that relating to the thickness of artery（thready pulse group,normal pulse group）of BAI of both sides were gradually advanced with a significant difference among groups（P0.0
