睡眠呼吸暂停对阴虚阳亢型高血压患者内皮依赖性血管舒张功能的影响

摘　　要：目的探讨睡眠呼吸暂停(OSA)对阴虚阳亢型高血压病患者内皮依赖性血管舒张功能(FMD)的影响。方法纳入阴虚阳亢型高血压患者101例,其中高血压伴OSA患者52例,不伴OSA患者49例,并用肱动脉超声技术评价内皮依赖性血管舒张功能。结果伴OSA的高血压患者夜间平均收缩压水平高于不伴OSA的高血压患者(P<0.05),中重度OSA患者夜间平均收缩压水平高于轻度OSA患者(P<0.05)。伴OSA高血压患者的FMD低于不伴OSA的高血压患者(P<0.05),中重度和轻度OSA患者的FMD比较差异无统计学意义(P>0.05)。结论 OSA患者夜间平均收缩压水平升高与夜间交感神经兴奋有关;血管内皮功能不全是高血压早期原发表现,在中重度的OSA高血压患者中内皮功能损伤程度已经没有明显区别,提示要关注早期内皮功能的损伤及其机制。Objective To observe the influence of obstructive sleep apnea（OSA） on brachial flow-mediated dilation（FMD） in hypertensive patients with Yin-deficiency and Yang-hyperactivity syndrome.Methods One hundred and one hypertensive patients with Yin-deficiency and Yang-hyperactivity syndrome The 24 hour ambulatory blood pressure（ABPM） and polysomnography（PSG）,were ptrformed 52 hypertensive patients with OSA,and 49 without OSA.The brachial flow-mediated dilation were measured.Results The nocturnal systolic blood pressure in hypertensive patients with OSA was higher than those without OSA（P0.05）,while FMD in hypertensive patients with OSA was lower than those without OSA（P0.05）.In hypertensive patients with OSA,the severe nocturnal systolic blood pressure was higher than mild and moderate ones（P0.05）,while there was no difference in FMD between the severe,mild and moderate ones（P0.05）.Conclusion The increase of nocturnal systolic blood pressure in OSA hypertensive patients may relate to the nocturnal hyperactivity of sympathetic nerve.There was no difference in FMD between the severe,mild and moderate OSA patients with hypertension.So that the endothelial dysfunction could be the early manifestation of hypertension,and it is important to pay attention to endothelial dysfunction at early stage.
