碎片状QRS波在高血压左室肥厚患者中的意义

摘　　要：目的探讨碎片状QRS波对高血压左室肥厚的诊断价值。方法选取2003年1月～2010年12月在大同市第四人民医院心内科住院,并确诊为高血压的患者208例,其中11例(9例心电图表现为左、右束支传导阻滞,2例起搏心律)被排除。其余197例经超声心动图检查确诊为左室肥厚的94例,将此94例作为研究组,103例非左室肥厚者作为对照组。结果心电图碎片状QRS波诊断高血压左室肥厚,其敏感性高于左室肥厚的电压标准,特异性略低。结论碎片状QRS波在高血压左室肥厚患者中容易出现,与左室电压改变相比,十二导联心电图碎片状QRS波诊断左室肥厚有更高的敏感性。Objective To investigate the significance of a fragmented QRS in patients with left ventricular hypertrophy in hypertension.Methods 208 patients,who were randomly selected,were diagnosed as HBP in Datong fourth hospital during 2003-2010.Because of bundle branch block（n=9） and paced rhythm（n=2）,11 patients were excluded.94 patients who were diagnosed as LVH by UCG,were regarded as study group,103 patients of non-left ventricular hypertrophy were used as control group.Results Compared with voltage criteria,the fQRS on a 12-lead ECG has higher sensitivity.Conclusion The fQRS is associated with regional myocardial ischemia,the fibrosis of cardiac tissue and scarring,so it is easy occurrence in left ventricular hypertrophy in hypertension.The fQRS has some diagnostic value for left hypertrophy in hypertension.
