摘 要:目的通过观察氨溴索对机械通气大鼠肺组织肺表面活性蛋白A(SPA)和髓过氧化物酶(MPO)表达水平的影响,探讨氨溴索对呼吸机所致肺损伤(VILI)的防治作用。方法Wistar大鼠24只,随机分为对照组、机械通气组(MV组)和氨溴索干预组(AMB组)。观察各组大鼠肺组织病理学改变,测定支气管肺泡灌洗液(BALF)中性粒细胞计数,采用考马斯亮蓝法测定BALF蛋白含量,采用MPO检测试剂盒测定BALF和肺组织MPO活性,采用反转录聚合酶链反应和免疫组织化学染色方法测定肺组织SPA mRNA及其蛋白表达。结果MV组大鼠BALF中性粒细胞计数、BALF和肺组织MPO活性较对照组和AMB组明显升高(P值均〈0.01),而肺组织SP-A mRNA及其蛋白表达水平较对照组和AMB组明显降低(P值均〈0.01);对照组上述指标与AMB组比较差异无统计学意义。相关性分析显示,肺组织SP—A水平与MPO活性呈负相关(r=-0.775,P〈0.05)。结论SPA和MPO在VILI的发生发展中具有重要作用,氨溴索可通过降低机械通气大鼠肺组织MPO活性,促进SP—A表达,对VILI有一定保护作用。Objective To explore the effects of ambroxol on ventilator induced lung injury (VILI) by analying the changes of surfaetant protein A (SP A) and myeloperoxidase (MPO) in lung of rats with mechanical ventilation. Methods Twenty four health Wistar rats were randomly divided into there groups: control group, mechanical ventilation group (MV group) and ambroxol pretreatment group (AMB group). The pathological changes of the lungs and the neutrophil counts in bronchoalveolar lavage fluid (BALF) were detected, the protein level in BALF was measured by brillant blue method. MPO activity in BALF and lung tissue was determined, the expressions of SP-A mRNA and protein in lung tissue were determined by reverse transcription polymerase chain reaction and immunohistochemistry staining. Results Compared with control group and AMB group,the neutrophil counts and MPO activity obviously increased (all P 〈0.01 ), and the expressions of SP-A mRNA and protein in lung obviously decreased (all P〈0.01) in MV group. There was no statistical significance between control group and AMB group on these indicators. Correlation analysis showed that the expression of SP-A was negatively correlated with the activity of MPO ( r =0. 775, P 〈0.05). Conclusions SPA and MPO play an important role in the development of VILI. Ambroxol may have protective effects on VILI by decreasing the activity of MPO and increasing the expressions of SPA mRNA and protein in lung of rats with mechanical ventilation.
