摘  要:目的探讨原发性高血压患者内皮型一氧化氮合酶(eNOS)基因G894T多态性与替米沙坦降压疗效及糖脂代谢的相关性。方法 2007年6月至2009年3月入选甘肃省东乡族270例原发性高血压患者,其中男性189例,女性81例,平均年龄(55.82±8.31)岁,安慰剂治疗两周后,服用替米沙坦单药4周,选取符合条件的高血压合并糖尿病患者40例继续服用替米沙坦到第10周,测定空腹血糖、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)及血尿酸的变化。抽提DNA,对高血压患者的eNOS第7外显子G894T进行基因多态性分型。结果 eNOS基因G894T多态性符合Hardy-Weinberg平衡定律;G、T等位基因的频率分别为0.8571和0.1429。经替米沙坦治疗4周后GG基因携带者的收缩压、舒张压下降大于T等位基因型携带者(P<0.05),40例高血压合并糖尿病患者治疗10周后空腹血糖、TG、TC、HDL-C、LDL-C、血尿酸较治疗前均有明显下降(P<0.01)。结论东乡族高血压患者eNOS基因G894T多态性与替米沙坦降压疗效有相关性,替米沙坦可同时改善。Objective To discuss the correlation between polymorphism of gene extron (G894T) of endothelial nitric oxide synthase (eNOS) and anti-hypertension effect of telmisartan and glucose-lipid metabolism in the patients of Dongxiang nationality with essential hypertension.Methods The patients (n=270) of Dongxiang nationality with essential hypertension male 189,female 81 and average age(55.82±8.31) were selected from Gansu province from June of 2007 to March of 2009.All patients were given dummy for 2 weeks,and then treated with telmisartan for 4 weeks.Of 270 patients 40 with hypertension combined with diabetes were selected and treated with telmisartan continuously until the 10th week.The changes of fasting plasma glucose,triglyceride (TG),total cholesterol (TC),high-density lipoprotein-C (HDL-C),low-density lipoprotein-C (LDL-C) and blood uric acid were observed and recorded.DNA was extracted and G894T polymorphism of eNOS was classified in the patients,and the levels of body mass index (BMI),fasting plasma glucose,TG and TC were detected.Results G894T polymorphism of eNOS was accorded with Hardy-Weingerh law and the frequency of G allele was 0.8571 and T allele,0.1429.After the treatment with telmisartan for 4 weeks the decreases of SBP and DBP were more significant in the carriers of GG gene than those in the carriers of T allele (P0.05).After the treatment for 10 weeks in 40 patients with hypertension combined with diabetes,the levels of fasting plasma,TG,TC,HDL-C,LDL-C and BUA decreased significantly compared with those before the treatment (P0.01).Conclusion There is a correction between G894T polymorphism of eNOS and anti-hypertension effect of telmisartan,and the level of glucose-lipid metabolism is improved in the patients of Dongxiang nationality with essential hypertension.
