高原地区幽门螺杆菌的分离培养方法

摘　　要：目的建立适合高海拔、低气压、低氧含量等高原环境特点的幽门螺杆菌(Hp)分离培养方法。方法随机收集186例知情同意的西藏军区总医院胃病患者胃窦、胃体、胃底部黏膜组织各1份,匀浆法研碎混匀后分别用常规法和根据高原环境特点改进的减量法、加氧法、加压加氧法同时进行培养,比较各培养方法的差异,同时分析培养结果与藏汉民族、性别、不同胃病间的相关性。结果各培养方法中,常规法阳性率最低(1.1%),减量法最高(38.2%)。常规法培养阳性率显著低于减量法、加氧法、加压加氧法(P<0.01)。Hp培养阳性率的差异在藏汉民族、性别间无统计学意义(P>0.05),而在不同胃病间具有统计学意义(P<0.05)。结论根据高原环境特点改进的减量培养方法更适合高原地区Hp的分离培养;Hp感染率的高低与藏汉民族、性别无关,而与不同种类胃病相关。Objective To establish a method of separation and cultivation for Helicobacter Pylori（Hp） which should be suitable for high altitude,low air pressure,low oxygen content of a plateau area features.Methods The samples of gastric mucous membrane from 186 patients were randomly collected and cultured in Lhasa City by routine method and three modified methods including decrement method,adding oxygen method,adding oxygen and pressure method based on the features of high altitude.The positive rates of the four different cultivation methods were compared and analyzed.The correlation between the cultured results and different ethnic people（Tibetan and Han）,gender and diseases.The Hp-infected rate in 1 315 patients was counted with the results of 14C test.Results Hp positive rate of routine cultural method was the lowest（1.1%）,and the decrement method was the highest（38.2%）.The difference of cultivation positive rate between routine cultural method and 3 modified methods of were statistically significant（P 0.01）.No statistically significant difference（P 0.05） showed between the cultivation positive rates and gender,Tibetan and Han ethnicity,while the difference of positive rates was significant among different digestive diseases（P 0.05）.Conclusions Decrement method should be more suitable for the growth of Hp in the plateau area.Hp infected rate was associated with different digestive diseases.
