控制性降压在颅内动脉瘤夹闭术麻醉中的应用

摘　　要：目的:探讨控制性降压在颅内动脉瘤夹闭术麻醉中的应用效果。方法:将在我院行颅内动脉瘤夹闭术的患者56例随机分为观察组和对照组,在常规麻醉的基础上对照组给予硝酸甘油实施控制性降压,观察组给予异丙酚实施控制性降压,评估降压效果。结果:控制性降压5min及15min后两组患者MAP水平均明显低于术前,两者分别比较差异具有统计学意义(P<0.05)。控制性降压5min及15min后两组MAP比较差异无统计学意义(P>0.05)。观察组停止控制性降压5min及15min后MAP水平均明显高于对照组,两者比较差异具有统计学意义(P<0.05)。观察组控制性降压后5min、控制性降压15min、停止降压后5min和停止降压后15min HR明显低于对照组,两者比较差异具有统计学意义(P<0.05)。结论:控制性降压在颅内动脉瘤夹闭术麻醉中具有积极的临床效果,临床医师应合理选择降压药物。Objective： To investigate the effect of controlled hypotension in anesthesia of clipping of intracranial aneurysm.Method： 56 cases with clipping of intracranial aneurysm were divided into the observe group and the control group.The control group received the controlled hypotension of nitroglycerin,and the observe group received the controlled hypotension of propofol.Result： The level of MAP in the two groups after 5min and 15min of controlled hypotension were lower than those before operation,and there was significant difference in them（P〈0.05）.There was no significant difference about level of MAP in the two groups after 5min and 15min in the two groups（P〈0.05）.The level of MAP in the observation groups after 5min and 15min of stopping controlled hypotension were higher than those in the control group,and there was significant difference in them（P〉0.05）.The level of HR in the observation groups after 5min and 15 min of controlled hypotension,5min and 15min of stopping controlled hypotension were lower than those in the control group,and there was significant difference between them（P〈0.05）.Conclusion： The effect of controlled hypotension in anesthesia of clipping of intracranial aneurysm is obvious,but the antihypertensive drugs should be choosen reasonably.
