下肢深静脉血栓的彩色多普勒超声诊断

摘　　要：目的探讨二维及彩色多普勒超声检查对下肢静脉血栓的诊断价值,总结最佳超声检查途径方法。方法回顾性分析69例下肢深静脉血栓患者血管壁、内膜、管腔内回声,并测量受累部位的血栓范围、彩色多普勒血流显像,观察血管内血流充盈情况及有无狭窄,同时利用Valsalva试验判断邻近静脉瓣功能。结果左下肢的发病率明显高于右下肢,超声表现为腔内有实性回声充填,探头加压管腔不能压瘪;彩色血流信号变细或无血流信号;脉冲多普勒示无血流或血流频谱不随呼吸而变化。结论彩色多普勒超声检查诊断下肢静脉血栓具有直观性、可靠性,是诊断下肢静脉血栓的首选方法;同时可通过动态观察其演变情况,有助于指导临床治疗。Objective To explore the diagnostic value of examining the thrombophlebity of the lower extremities by two-dimensional and Color Doppler Ultrasound, so as to find out the best ultrasonic way. Methods In 69 patients with thrombosis in deep vein of lower limbs, thrombosis ranges, color Doppler flow imaging, blood stream, and resonance from vessel walls, endomembranes or luminas were recorded, the venous valve functions were evaluated by Valsalva＇ s maneuver, and the data was analyzed retrospectively. Results.. The most common site of venous thrombosis was the left lower limbs, the vein lumens which were full of solid echo did not vanish with pressing the transducer;thin blood flow or no blood flow was observed by Color Doppler. Slow velocity was detected or the velocity was not changed with deep breath in Pulse Doppler. Conclusion Because of its sensitivity and intuitiveness, color Doppler Ultrasound was one of the best reliable diagnose for thrombosis in deep vein of lower limbs ; and more importantly, color Doppler Ultrasound can also help to direct treatment for the thrombosis in deep vein of lower limbs.
