经尿道冷刀切开联合耻骨上小切口术式治疗复杂性尿道狭窄

摘　　要：目的总结腔镜技术在治疗复杂性尿道狭窄中的应用及价值。方法复习18例复杂尿道狭窄患者，自耻骨上小切口或原膀胱造瘘口以尿扩器或手指等作导引，配合尿道镜下以冷刀切开尿道狭窄段；术后保留导尿均4周以上，拔管后例行尿道扩张。结果随访6-100个月，2例出现程度不等尿失禁及残余尿，需长期定期尿道扩张，余16例排尿通畅，除原3例术前已存在阴茎勃起功能障碍外，余术后均未发生明显阳痿；附睾炎1例。结论我们采用耻骨上小切口联合腔镜技术治疗复杂性尿道狭窄具有直视下进行、创伤小、安全、有效、可重复性；避免假道形成等优点，取得满意效果。The combined operation of transurethral incision and little cut above pubis on complex urethroslenosisLIANG Qing, CHEN Bo, DONG Bingzheng, JIANG Bo, ZHANG Wenda, WU Tianlin, HAN ConghuiThe Central Hospital of Xuzhou, Xuzbou 221009 Objective： To explore the application and value of endoscopic technique in complex urethrostenosis. Methods： To review 18 cases with complex urethrostenosis, discissioning the narrow-urethra with co＇o-scalpel through urethral endoscope, guided by expandor or the finger insertioning from posterior urethra by little cut above pubis or bladder orificium fistulae. Retentioning catheterization above 4 weeks postoperation, and then making urethral sounding after extubale. Results： Following up the patients for 6-100 months, we found that there were 2 patients with the complecations of aniso-degree incontinentia urina or with retaining residual urine who should operate the urethral sounding regularly for long tenn. The 16 rest patients urinated easily and smoothly. Except for the three patients who already had Erectile dysfunction before our operation, no patients occurred obviously impotence. In the 18 patients only one patient had epididymitis. Conclusions： The operation of combining transurethral incision with little cut above pubis on complex urethrostenosis has many merits such as safety, little wound, utility, repeatability, preventing false passage and so on. Therefore, it would be a perspective operation for complex urethrostenosis.
